บทคัดย่อ
วัตถุประสงค์: เพื่อประเมินและเปรียบเทียบระดับสมรรถนะทางด้านงานบริการส่งมอบและให้คำแนะนำในการใช้ยาที่มีอยู่จริงและที่คาดหวังของผู้ให้บริการในโรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) จังหวัดกาญจนบุรี และ เพื่อรวบรวมปัญหา อุปสรรค และข้อเสนอแนะในการพัฒนาสมรรถนะดังกล่าว วิธีการ: การวิจัยนี้เป็นการศึกษาแบบผสมผสาน ในระยะที่ 1 เป็นการศึกษาเชิงปริมาณที่ใช้วิธีสำรวจแบบภาคตัดขวางด้วยแบบสอบถาม กลุ่มตัวอย่าง คือ ผู้ให้บริการที่รับผิดชอบหลักด้านการส่งมอบและให้คำแนะนำในการใช้ยาใน รพ.สต. 144 คน ระยะที่ 2 เป็นการศึกษาเชิงคุณภาพที่ใช้วิธีการสัมภาษณ์เชิงลึกในผู้ให้ข้อมูล 10 คนจากการศึกษาระยะที่ 1 ผลการวิจัย: ระยะที่ 1 แบบสอบถามถูกตอบกลับครบทั้ง 144 ฉบับ ตัวอย่างต้องการพัฒนาสมรรถนะ ดังนี้ 1) กระบวนการตรวจสอบความถูกต้องก่อนส่งมอบ 5 ด้าน ได้แก่ การวินิจฉัยความรุนแรงของปัญหาที่เกี่ยวเนื่องกับการใช้ยา (drug related problems: DRPs) การค้นหา DRPs การแก้ไข DRPs การตรวจสอบความถูกต้องด้านเทคนิคในการบริหารยา และด้านขนาดยา 2) กระบวนการส่งมอบและให้คำแนะนำในการใช้ยา 7 ด้าน ได้แก่ การบันทึกอุบัติการณ์ความคลาดเคลื่อนทางยา การประเมินและการจัดการกรณีผู้ป่วยแพ้ยา การบันทึกแจ้งเตือนผู้ป่วยใช้ยาที่มีความเสี่ยงสูงในคอมพิวเตอร์ การบันทึกแจ้งเตือนผู้ป่วยแพ้ยาในคอมพิวเตอร์ การตรวจสอบความร่วมมือในการใช้ยา การบันทึกและทำสัญลักษณ์ผู้ป่วยแพ้ยา และการให้คำแนะนำในการใช้ยาโดยวาจาร่วมกับการใช้เอกสารประกอบ 3) กระบวนการประเมิน DRPs และการส่งต่อ 4 ด้าน ได้แก่ การค้นหาปัญหาและปัจจัยที่มีผลต่อการเกิด DRPs ที่บ้าน การค้นหา DRPs และปัจจัยที่มีผลต่อการเกิด DRPs การวินิจฉัยความรุนแรงของ DRPs และการส่งต่อ และการประเมินผลการแก้ DRPs คะแนนสมรรถนะที่มีอยู่จริงและระดับสมรรถนะที่คาดหวังในทุกประเด็นที่กล่าวมามีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ ระยะที่ 2 ปัญหาและอุปสรรคในการปฏิบัติงานส่งมอบและให้คำแนะนำในการใช้ยา ได้แก่ การขาดองค์ความรู้ ทักษะ ประสบการณ์ ความเข้าใจด้านภาษา และการเชื่อมโยงข้อมูลผู้ป่วย รวมถึงความไม่เพียงพอของบุคลากร คู่มือ แนวปฏิบัติหรือเอกสารทางวิชาการ ข้อเสนอแนะในการพัฒนาสมรรถนะด้านการส่งมอบและให้คำแนะนำในการใช้ยา ได้แก่ การจัดอบรมให้ความรู้ การสนับสนุนบุคลากร คู่มือ แนวปฏิบัติหรือเอกสารทางวิชาการ ฉลากยารูปภาพ ฉลากยาภาษาต่างประเทศ คู่มือการสื่อสารภาษาต่างประเทศ และการเชื่อมโยงข้อมูลที่ดี สรุป: ผู้ให้บริการใน รพ.สต.ยังต้องการพัฒนาสมรรถนะด้านการส่งมอบและให้คำแนะนำในการใช้ยาตามแนวทางการดำเนินงานเภสัชกรรมปฐมภูมิและแนวทางการพัฒนา รพ.สต.ติดดาวในหลายด้าน ดังนั้นจึงควรจัดอบรมและจัดหาปัจจัยสนับสนุนการปฏิบัติงานที่สอดคล้องกับความต้องการ เพื่อเพิ่มประสิทธิภาพในการปฏิบัติงาน
