บทคัดย่อ
วัตถุประสงค์: เพื่อพัฒนาระบบการดูแลผู้ป่วยจิตเวชเรื้อรังในชุมชนร่วมกับทีมสหวิชาชีพโดยศึกษาเฉพาะผู้ป่วยจิตเภทในโรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) 3 แห่งของอำเภอบ้านแท่น จังหวัดชัยภูมิ วิธีการ: การศึกษานี้เป็นการวิจัยเชิงปฏิบัติการ ผู้เข้าร่วมงานวิจัยได้แก่ บุคลากรทางการแพทย์ 8 คน อสม. 4 คน ผู้ป่วยจิตเภท 13 คน และญาติผู้ดูแล 6 คน การวิจัยดำเนินการตั้งแต่ ตุลาคม 2560 ถึงพฤษภาคม 2561 โดยแบ่งออกเป็น 2 ระยะ คือ 1) การค้นหาและวิเคราะห์ปัญหาของผู้ป่วย 2) การพัฒนาระบบการดูแลผู้ป่วยจิตเภทในชุมชน ผลการวิจัย: ผู้ป่วยโรคจิตเภทในชุมชนมีความร่วมมือจากการใช้ยาอย่างสม่ำเสมอเพิ่มขึ้นจากร้อยละ 46.2 เป็นร้อยละ 61.5 อาการไม่พึงประสงค์ที่รบกวนชีวิตประจำวันลดลง เช่น อาการง่วงนอนมากทุกวันที่พบจากร้อยละ 46.2 เป็นไม่พบผู้ป่วยที่มีอาการดังกล่าว ในระหว่างการศึกษาไม่พบผู้ป่วยจิตเภทที่มีอาการกำเริบจนก่อให้เกิดอันตรายร้ายแรงต่อชีวิต ร่างกาย หรือทรัพย์สินของตนเองหรือผู้อื่น สรุป: การพัฒนาระบบการดูแลผู้ป่วยจิตเภทในชุมชนโดยทีมสหวิชาชีพสามารถแก้ปัญหาที่เกิดจากการใช้ยาและความไม่ร่วมมือในการใช้ยาของผู้ป่วย ทำให้ผู้ป่วยได้รับการดูแลอย่างต่อเนื่อง ไม่มีอาการกำเริบ และอยู่ร่วมกับชุมชนได้
