ผลของการให้การบริบาลเภสัชกรรมแบบไร้รอยต่อของผู้ป่วยในคลินิก โรคปอดอุดกั้นเรื้อรัง โรงพยาบาลพหลพลพยุหเสนา จังหวัดกาญจนบุรี

บทคัดย่อ

วัตถุประสงค์:  เพื่อประเมินผลการให้บริบาลเภสัชกรรรมแบบไร้รอยต่อ โดยเปรียบเทียบผลลัพธ์ด้านการรักษาโรคปอดอุดกั้นเรื้อรังและผลลัพธ์ด้านคุณภาพชีวิตระหว่างกลุ่มศึกษาที่ได้รับการบริบาลเภสัชกรรมกับกลุ่มควบคุมที่ได้รับการบริการตามปกติ  วิธีการ: การวิจัยนี้เป็นการศึกษาแบบนำร่อง ผู้วิจัยจัดแบ่งผู้ป่วย 60 รายด้วยวิธีสุ่มออกเป็นกลุ่มศึกษาและกลุ่มควบคุม กลุ่มละ 30 คน กลุ่มศึกษาได้รับการบริบาลเภสัชกรรมทั้งหมด 4 ครั้ง เริ่มตั้งแต่ผู้ป่วยเข้ามาพักรักษาตัวในโรงพยาบาล ตลอดจนกลับมารักษาต่อเนื่องในคลินิกโรคปอดอุดกั้นเรื้อรังแบบผู้ป่วยนอก กลุ่มควบคุมได้รับการบริการตามปกติตามรูปแบบของโรงพยาบาล ผู้ป่วยทั้ง 2 กลุ่มได้รับการประเมินผลลัพธ์ด้านต่าง ๆ หลังจากถูกจำหน่ายออกจากโรงพยาบาลประมาณ 3 เดือน ผลการวิจัย: เมื่อสิ้นสุดการวิจัยพบว่า กลุ่มศึกษามีจำนวนครั้งของการเข้าใช้บริการในแผนกฉุกเฉินและการเข้าพักรักษาตัวในโรงพยาบาลน้อยกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (P=0.002, P=0.006) มีภาวะหายใจลำบาก (mMRC dyspnea score) น้อยกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (P=0.022) มีค่าสมรรถภาพการทำงานของปอด (PEFR) สูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (P=0.025) และมีคุณภาพชีวิต (CAT score) ดีกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (P<0.001) สรุป: การบริบาลเภสัชกรรมแบบไร้รอยต่อ สามารถลดการเข้าใช้บริการในแผนกฉุกเฉิน ลดการเข้าพักรักษาตัวในโรงพยาบาล และลดภาวะหายใจลำบากของผู้ป่วยได้ ทั้งนี้ยังสามารถเพิ่มสมรรถภาพการทำงานของปอดและนำไปสู่คุณภาพชีวิตที่ดีขึ้น
