บทคัดย่อ
ภูมิหลัง: ภาวะไส้ติ่งแตกสัมพันธ์กับภาวะแทรกซ้อนหลังผ่าตัดและระยะเวลานอนโรงพยาบาลที่เพิ่มขึ้น ข้อมูลร่วมสมัยจากโรงพยาบาลระดับภูมิภาคมีความสำคัญต่อการประเมินภาระโรคและการพัฒนาคุณภาพบริการ
วัตถุประสงค์: เพื่อศึกษาสัดส่วนของภาวะไส้ติ่งแตกที่ยืนยันระหว่างผ่าตัด ลักษณะทางคลินิกและผลลัพธ์หลังผ่าตัด ตลอดจนสำรวจความสัมพันธ์ระหว่างตัวแปรก่อนผ่าตัดที่มีบันทึกเป็นกิจวัตรกับภาวะไส้ติ่งแตกในผู้ป่วยที่ได้รับการผ่าตัดไส้ติ่ง ณ โรงพยาบาลลำปาง
วัสดุและวิธีการ: การศึกษาตามรุ่นย้อนหลังในผู้ป่วยอายุ ≥15 ปีที่ได้รับการผ่าตัดและยืนยันการวินิจฉัยไส้ติ่งอักเสบ ระหว่างวันที่ 1 ตุลาคม 2566 ถึง 30 กันยายน 2567 เก็บข้อมูลประชากรศาสตร์ ข้อมูลทางคลินิก ผลตรวจทางห้องปฏิบัติการ และการรักษาจากเวชระเบียน แล้วเปรียบเทียบระหว่างกลุ่มไส้ติ่งแตกกับไม่แตก วิเคราะห์ความสัมพันธ์หลังปรับตัวแปรร่วมด้วยการถดถอยโลจิสติกพหุตัวแปรเชิงสำรวจ และจัดการข้อมูลสูญหายด้วยการแทนค่าข้อมูลสูญหายแบบพหุด้วยสมการลูกโซ่
ผลการศึกษา: ผู้ป่วย 443 รายเข้าเกณฑ์การศึกษา โดย 153 รายพบไส้ติ่งแตกยืนยันระหว่างผ่าตัด คิดเป็นร้อยละ 34.5 กลุ่มไส้ติ่งแตกพบไข้หลังผ่าตัด (ร้อยละ 69.9 เทียบกับ 38.6; p<0.001) และการติดเชื้อบริเวณแผลผ่าตัด (ร้อยละ 13.7 เทียบกับ 1.4; p<0.001) บ่อยกว่า การวิเคราะห์พหุตัวแปรเชิงสำรวจพบว่า เพศชาย (aOR 1.58; 95% CI 1.04–2.39) อายุเกิน 60 ปี (aOR 3.11; 95% CI 2.01–4.81) และอุณหภูมิร่างกายแรกรับที่เพิ่มขึ้นทุก 1 องศาเซลเซียส (aOR 1.36; 95% CI 1.09–1.71) สัมพันธ์กับโอกาสพบภาวะไส้ติ่งแตกที่สูงขึ้น
สรุป: ภาวะไส้ติ่งแตกคิดเป็นร้อยละ 34.5 ของผู้ป่วยที่ได้รับการผ่าตัดไส้ติ่ง และสัมพันธ์กับภาระภาวะแทรกซ้อนหลังผ่าตัดที่สูงขึ้น ความสัมพันธ์ของเพศ อายุ และอุณหภูมิแรกรับเป็นผลเชิงสำรวจและไม่ควรใช้เป็นเกณฑ์พยากรณ์หรือจัดลำดับความเร่งด่วนเพียงลำพัง เนื่องจากไม่มีข้อมูลสำคัญด้านการตรวจร่างกาย ภาพถ่ายรังสี การส่งต่อ และช่วงเวลาการดูแลอย่างครบถ้วน ควรเก็บข้อมูลดังกล่าวแบบไปข้างหน้าอย่างเป็นมาตรฐานเพื่อระบุปัจจัยที่ปรับเปลี่ยนได้สำหรับการพัฒนาคุณภาพในบริบทของโรงพยาบาล
