Delirium or Acute confusion is a older person’ s problem that overlooked, it is more related with physical illness than reflected to physical of patient. Delirium is a state of emergency , must be take care both physical illness and psychosocial to prevent complications as a cognitive impairment and long-term length of stay in hospital. Patients should be encouraged by having physical activity as fast as possible to prevent sarcopenia because might lead to long-term care in hospital and difficult to rehabilitation. Collaboration between nurses and health care team to consider of taking medication, especially the patient should be received medicine low doses and stop taking its as soon as possible. The purpose of this article is to educate and understand for nursing care of delirium who have intensive care, undergoing surgery, and palliative care. Providing the details of risk factors for delirium, tools used to assessment delirium, and nursing care of delirium in older person.
