บทคัดย่อ

การสั่งใช้ยาต้านการแข็งตัวของเลือดชนิดกินที่ออกฤทธิ์โดยตรง (direct oral anticoagulants: DOACs) ในผู้ป่วยภาวะหัวใจห้องบน สั่นพลิ้ว แผนกผู้ป่วยนอก โรงพยาบาลพุทธชินราช พิษณุโลก พบปัญหา การสั่งใช้ยา ไม่เหมาะสม การติดตามไม่เพียงพอ และรายงานเหตุการณ์ ไม่พึงประสงค์รุนแรง จึงได้พัฒนาระบบสนับสนุน การตัดสินใจทาง คลินิกด้วยคอมพิวเตอร์ (computerized clinical decision support  system: CCDSS) เพื่อเพิ่ม ความเหมาะสมและความปลอดภัยของการ ใช้ยา การศึกษาแบบกึ่งทดลองครั้งนี้เพื่อเปรียบเทียบความเหมาะสม และ ความปลอดภัยของการสั่งใช้ DOACs ในผู้ป่วย AF ก่อนและหลัง การใช้ CCDSS ณ แผนกผู้ป่วยนอก โรงพยาบาล พุทธชินราช พิษณุโลก โดยเก็บข้อมูลย้อนหลังระหว่างวันที่ 1 ตุลาคม พ.ศ. 2567 ถึงวันที่ 30  เมษายน พ.ศ. 2568 และติดตามต่อเนื่อง 3 เดือนหลังเริ่มใช้ CCDSS พบผู้ป่วยทั้งหมด 202 รายได้รับ Dabigatran 91 ราย (ร้อยละ 45) และ Apixaban 111 ราย (ร้อยละ 55) ภายหลังการใช้ CCDSS การสั่งยาที่ สอดคล้องตามคำแนะนำมาตรฐานเพิ่มขึ้น ร้อยละ 7.9 ความเสี่ยงต่อการสั่งยาไม่สอดคล้องตามคำแนะนำลดลงอย่างมีนัยสำคัญทางสถิติ (aRR = 0.52, 95% CI: 0.37-0.73, p < 0.001) อัตราการกลับมานอนโรงพยาบาลภายใน 30 วันมีแนวโน้มลดลงร้อยละ 2.4 (aRR = 0.48, 95% CI: 0.10-2.18, p = 0.339) รายงานอันตรกิริยาระหว่างยาทุกกรณีได้รับการยืนยันเหตุผลเชิงระบบ สรุปได้ว่า CCDSS มีประสิทธิผลสนับสนุนการจัดการยาเชิงระบบอย่างเหมาะสมและปลอดภัย
