บทคัดย่อ

โรคไม่ติดต่อเรื้อรังเป็นปัญหาสุขภาพอันดับหนึ่งของโลกและของประเทศไทย ในปี พ.ศ. 2561 ศูนย์สุขภาพชุมชนเมือง อำเภอเมือง พิษณุโลก มีผู้รับบริการ 44,442 ครั้ง (19,725 คน) ซึ่งเป็นโรคไม่ติดต่อ เรื้อรังร้อยละ 60.3 ทำให้แออัด การเข้าถึงการคัดกรองน้อยกว่าเป้า หมาย ชุมชนมีส่วนร่วมในการดูแลสุขภาพตนเองน้อย การวิจัยเชิงปฏิบัติ การแบบมีส่วนร่วมครั้งนี้ดำเนินการระหว่างเดือนมกราคมถึงเดือน  ธันวาคม พ.ศ. 2562 โดยมีวัตถุประสงค์เพื่อพัฒนารูปแบบการคัดกรอง สุขภาพในชุมชนเมืองโดยอาสาสมัครสาธารณสุขประจำหมู่บ้าน (อสม.) และทีมหมอครอบครัวโดยศึกษาใน อสม. 20 คนที่สมัครใจเข้าร่วมวิจัย และประชาชนที่อายุ 35 ปีขึ้นไปที่อาศัยอยู่ในเขตทีมหมอครอบครัวทีมที่ 4 อำเภอเมืองพิษณุโลก 135 คนซึ่งสุ่มด้วยการจับฉลากและยินยอม เข้าร่วมวิจัย นำเสนอเป็นจำนวน ค่าร้อยละ และค่ามัธยฐาน (Q1, Q3)  เปรียบเทียบข้อมูลระหว่างการเข้ารับบริการ 6 เดือนแรกและ 6 เดือน หลังด้วยสถิติ Wilcoxon signed rank ผลการศึกษาได้รูปแบบการคัด กรองและตรวจติดตามความดันโลหิตและน้ำตาลในเลือดในชุมชนที่ส่ง เสริมให้ผู้รับบริการเข้าถึงบริการ ค่ามัธยฐาน (Q1, Q3) ของจำนวนครั้ง ที่มารับบริการใน 6 เดือนหลังเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ [145.5  (138, 209) ครั้งต่อเดือนและ 254.5 (202, 314) ครั้งต่อเดือนตามลำดับ (p = 0.027)] โดยทั้งประชาชนผู้รับบริการและผู้ให้บริการ (อสม.) พึง พอใจในระดับมากที่สุด จึงควรนำรูปแบบที่พัฒนานี้มาวัดผลลัพธ์ทาง คลินิกต่อไป
