Acute Poststreptococcal Glomerulonephritis: Prevalence and Risk Factors Associated with Complications in Saraburi Hospital

Abstract

Acute poststreptococcal glomerulonephritis (APSGN) is a common form of glomerulonephritis in children. The prognosis is excellent in most cases. However, some may suffer from acute kidney injury. This retrospective research collected data from medical records of children who were diagnosed with APSGN and admitted at Saraburi Hospital between 1 January 2013 and 31 December 2022 to evaluate prevalence, clinical manifestations, investigations, outcomes, and risk factors associated with complications of APSGN. Statistical analyses were conducted using the Chi-square test and Fisher’s exact test.

Ninety-four patients were enrolled. The mean age was 8.7 years old, and the majority was male (69.2%). The peak was August. The preceding upper respiratory tract infection was 37.2%. Clinical manifestations were hypertension (96.2%) An elevated in anti-DNAse B and ASO titer were detected in 90.3% and 84.9%, respectively. Complications were acute kidney injury (AKI) (70.2%) and rapidly progressive glomerulonephritis (RPGN) (12.8%). Gross hematuria was significantly associated with AKI (p-value 0.048). Gross hematuria and urine protein ≥ 3+ were significantly associated with RPGN (p-value 0.046 and 0.001, respectively).

In conclusion, gross hematuria was a significant risk factor for AKI, and gross hematuria and urine protein ≥ 3+ were significant risk factors for RPGN.
