Development of a Service System for Women with an Unplanned Pregnancy and Safe Abortion in Chanthaburi Province

The service system for unplanned pregnant women and the termination of pregnancy was developed to mitigate the adverse effects of unsafe abortions. However, it was found that this service system lacked internal coordination to facilitate a rapid and safe abortion process in all dimensions according to the Ministry of Public Health's declaration. This participatory action research aimed to examine the situation of the service system for unplanned pregnant women and safe abortion practices in accordance with the amendments to the Criminal Code, and to develop a service system for unplanned pregnant women and safe abortion practices. The research was conducted in four stages: 1) planning a change, 2) acting, 3) observing, and 4) reflecting. The tools used included semi-structured interviews, focus groups, quantitative data analysis using descriptive statistics, and qualitative data analysis using content analysis.
The results of developing the service system for unplanned pregnant women and safe abortion practices indicated several obstacles to service provision. These included difficult access to services, lack of psychological support and alternative counseling, and insufficient post-abortion care. Success factors included promoting knowledge and understanding among service recipients to reduce abortion stigma from service providers, developing communication and referral channels to help unplanned pregnant women access services more easily and quickly, providing counseling and psychological support to empower women in decision-making and post-abortion care, and establishing a continuum of care to ensure ongoing health services and preventive follow-ups to reduce the risk of future unplanned pregnancies.
However, limitations of the developed service system included that some women still lacked understanding of their rights and available services, potentially hindering full access to services. Additionally, pregnant women in remote areas faced barriers to accessing services due to travel and resource constraints. This study provided insights into developing a safe abortion service system and highlighted significant improvements in service accessibility and quality. Nonetheless, it also pointed out remaining limitations that needed to be addressed to fully meet the needs of all women.
