The Diagnostic Accuracy of Sonography in the Evaluation of Acute appendicitis in Sing Buri hospital.
Abstract
Purpose: The purpose of this study was to evaluate the sensitivity, specificity, positive predictive value and negative predictive value of ultrasound 5-point scale in the diagnosis of acute appendicitis.
Materials and Methods: A total of 225 patients (82 male and 143 female; age average, 36.21 years) suspected of having acute appendicitis were retrospectively and prospectively included in the study and reviewed. The Alvarado score and the ultrasound 5-point scale were calculated from data in out-patient cards and Picture Archiving and Communication System (PACS). The sensitivity, specificity, positive predictive value andnegative predictive value of the Alvarado score and the ultrasound 5-point scale were assessed considering surgical findings and clinical follow-up as reference standard.
Results: The ultrasound 5-point scale had higher sensitivity, positive predictive value and negative predictive value as compare with the Alvarado score.
Conclusions: The ultrasound 5-point scale had high sensitivity, positive predictive value and negative predictive value and should be considered as a further investigation in the case that suspected acute appendicitis but having equivocal clinical findings and laboratory examination.
