The nursing of Intermediate care with Family Physician Thawung Hospital, LopBuri: case study

Introduction: This case study describes the intermediate care of patients with cerebrovascular disease (post-critical stroke). The objective was to enhance knowledge and skills of intermediate care.
Methods: This study investigated a post-critical stroke patient at ThaWung Hospital, Lopburi, Thailand. Follow-ups were continuously conducted during the golden period for stroke rehabilitation from October 2020 – March 2021.
Results: A patient came to the hospital with left side weakness and dysarthria. After checking the muscle power, it was found that the left side score was lower than the right side (Muscle power: Rt. UE&LE grade V, Lt. UE grade I, Lt. LE grade II). The Barthel Activity of Daily Living (ADL) Index score was 45 points. After treatment, the muscle power score on the left side increased (Muscle power: Rt. UE&LE grade V, Lt. UE grade II+, Lt. LE grade IV+) and the Barthel ADL Index scores was 90 points. The patient could walk by herself without a tripod cane.
Conclusion The result revealed that the nursing care for a patient with ischemic stroke during the golden period was important because the phase of brain neurons rapidly recovered. These could improve muscle power and sensation. Not only are family medicine processes effective but also Chinese traditional acupuncture treatments. As a result, the patient has a better quality of life.
