Nursing care of liver injury with Respiratory failure: Case study

Abstract

A case study was Thai male, 22 years old. The patient had accidentally prior to hospital caused by vehicle collision. The primary assessment showed that the patient had pain and guarding at abdomen, abrasion wound at left chest, restlessness, pale, and polar. The special investigation with CT whole abdomen showed liver injury grade 3 with large hemoperitoneum. The patient was diagnosed blunt abdominal injury with hemodynamic unstable, and right lung pneumo-hemothorax. The surgeon decided to treat with the operation method by exploratory laparotomy with perihepatic temporary abdominal packing.

The postoperative phase was treated by packed red cell (PRC), ventilator support, and intercostal chest drainage at right chest. Afterwards, the patient found that the blood test and chest x-ray was improved in 48 hours. The patients were staying in ICU for 4 days and can also release the intercostal chest drainage and ventilator support. Subsequently, the patient was moved to the general surgery department. The patient condition was better and could be discharged, the length of stay were 13 days.

Nursing care of patient with liver injury should acute and urgent care. The nurse was staffs who closed up to the patient and provided the correct nursing for fixing the problem of patient in crisis and before surgery. The nursing care would correspond to the treatment plan of doctor for helping patients immediately.
