Oral Health Care Access by Dental Personnel among Intermediate Care Patients at Somdet Prayanasungworn Hospital, Chiang Rai: Analysis of Service Processes and Systemic Recommendations

Patients undergoing intermediate care (IMC) are at an elevated risk of oral health problems due to mobility limitations and dependence on caregivers. However, the extent of oral health care received from dental personnel in Thai community hospitals remains unclear. This retrospective mixed-methods descriptive study aimed to examine oral health care access by dental personnel in the IMC system, identify process gaps, and synthesize systemic recommendations from multidisciplinary team consultation among IMC patients at Somdet Prayanasungworn Hospital, Chiang Rai, during fiscal years 2023–2025.The sample for quantitative research consisted of 391 care episodes from 368 patients; the primary outcome was documented oral health care by dental personnel within 90 days of registration, analyzed descriptively with 95% confidence intervals.The sample for qualitative research included 16 purposively selected multidisciplinary team members from nine professions, analyzed using directed content analysis guided by the Donabedian and Levesque's frameworks.
The results revealed that only 6.4% of care episodes had documented oral health care by dental personnel within 90 days (95% CI 4.4–9.3). Most services occurred post-discharge (64.0% within 31–90 days; median 36 days, IQR 29–68). The bedridden group (Barthel Index: BI 0–4) had the lowest access at 1.4%, versus 10.7% (home-bound, BI 5–11) and 6.6% (socially active, BI ≥12). Multidisciplinary team consultation identified five process gaps: workflow, documentation, communication, resources, and training, alongside structural barriers including an undefined dental role in IMC standards and the lack of oral health reimbursement codes in the IMC payment system.
A dual-pathway service model was proposed: ward-based dental consultation from the first day of admission and proactive community care for patients discharged directly home or with mobility limitations, linked by an automated IMC-registration alert. These findings suggest the need to integrate oral health care into the IMC system at the service-unit, community-network, and policy levels.
