Factors Associated with Quality of Life among Patients with Advanced Cancer Receiving Palliative Care at Phayao Hospital

Cancer is one of the major noncommunicable diseases (NCDs) and a significant public health problem. Although advances in medical treatment have improved survival, advanced cancer is often incurable and associated with substantial physical and psychological suffering. Palliative care plays a crucial role in alleviating suffering and improving quality of life. This descriptive cross-sectional study aimed to examine factors associated with quality of life and the level of quality of life among patients with advanced cancer receiving palliative care. The participants were 120 advanced cancer patients receiving palliative care at the inpatient and outpatient departments of Phayao Hospital between January and October 2025. Data were collected through structured interviews using a demographic questionnaire, a disease and treatment record form, the Palliative Performance Scale, and the EORTC QLQ-C30 Version 3.0. Data were analyzed using descriptive statistics and correlation coefficients.
The results showed that the participants were nearly equally distributed by sex, with a mean age of 70 years (Mean = 69.97, SD = 10.55). Overall quality of life was at a moderate level (Mean = 47.50, SD = 27.30). The PPS score was positively correlated with overall quality of life (p<.001). Functional quality-of-life domains, including physical, role, emotional, cognitive, and social functioning, were positively correlated with overall quality of life (p<.001). In contrast, symptom burden was negatively correlated with overall quality of life. Fatigue, pain, and insomnia showed moderate negative correlations, whereas nausea/vomiting, dyspnea, appetite loss, constipation, diarrhea, and financial difficulties showed weak negative correlations (p<.05). These findings suggest that promoting functional abilities and managing major symptoms, particularly fatigue, pain, and insomnia, should be integrated into palliative care to improve quality of life in patients with advanced cancer.
