Development of a Community-Based Healthcare Model for Patients with Diabetes Mellitus and Hypertension in PathumThani Province

Article presents the development of a community-based health care model for patients with diabetes mellitus and hypertension. This study aimed to develop, implement, and evaluate a community-based healthcare model for patients with diabetes mellitus and hypertension. Using participatory action research combined with a quasi-experimental design, 50 participants from Rangsit Subdistrict, Thanyaburi District, Pathum Thani Province engaged in a 24-week self-management program. The model was grounded in Orem’s Self-Care Theory, self-management theory, and Thai traditional medicine concepts. Data were collected through questionnaires, interviews, and clinical records of blood glucose and blood pressure. Quantitative analysis employed descriptive statistics and paired t-tests, while qualitative data were examined using content analysis. Results showed that most participants were female and aged 60 years or older. Post-intervention, mean knowledge scores increased significantly from 7.68 (SD=0.96) to 9.58 (SD=0.54), and health behavior scores improved from 3.19 (SD=0.52) to 4.31 (SD=0.14) (p<.001). Clinical outcomes demonstrated reductions in blood glucose from 130.42 (SD=27.76) mg/dl to 123.38 (SD=13.56) mg/dl, and blood pressure from 147.66/91.04 (SD=3.20/3.30) mmHg to 139.30/80.57 (SD=1.97/1.33) mmHg. Overall satisfaction with the model was high, particularly regarding family support. In conclusion, the integrated multi-approach combining community-based practices and self-management effectively improved knowledge, health behaviors, and clinical outcomes, supporting sustainable lifestyle changes among patients with diabetes mellitus and hypertension.
