Abstract

The purposes of this research were: (1) diagnose problems Health care of the elderly hypertension. (2) Develop a community-based health care model for hypertension. And try the model created. (3) Evaluate and improve the model. The study process consisted of 3 steps: (1) Diagnosis of the problem. The sample consisted of 173 hypertensive elderly persons and an in-depth interview with the public health officers and community leaders. (2) Develop a plan of activities by 10 community health network partners using participatory planning process, 30 hypertension treatment programs. (3) Evaluate and improve the model. By comparison, knowledge about hypertension, Practice of health behavior between before and after trial, the satisfaction of elderly patients with hypertension and members of the health network. Data were analyzed using percentage, mean, standard deviation Analyze the relationship of variables using chi-square, Pearson's correlation coefficient and t-test. Found that after the trial. Sample Have knowledge in health care. There are higher health care behaviors than before the trial. The mean of health status was blood lipid (P <0.05). Blood pressure, weight, height and waist circumference better and higher than before trial. It was found that the developed model was effective in helping elderly people with high blood pressure. Therefore, it should be applied to the elderly with hypertension.
