การพัฒนารูปแบบการดูแลสุขภาพผู้ป่วยโรคเบาหวานและโรคความดันโลหิตสูงตามวิถีชุมชนมีวัตถุประสงค์เพื่อพัฒนา ทดลองใช้ และประเมินผลรูปแบบการดูแลสุขภาพผู้ป่วยโรคเบาหวานและโรคความดันโลหิตสูงตามวิถีชุมชน โดยใช้การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม และการวิจัยกึ่งทดลองในกลุ่มตัวอย่าง 50 คน จากชุมชนตำบลรังสิต อำเภอธัญบุรี จังหวัดปทุมธานี บนพื้นฐานแนวคิดทฤษฎีการดูแลสุขภาพตนเองของโอเร็ม ทฤษฎีการจัดการตนเอง และแนวคิดการแพทย์แผนไทยมาประยุกต์ โดยดำเนินกิจกรรมการจัดการตนเองจำนวน 24 สัปดาห์ เก็บรวบรวมข้อมูลด้วยแบบสอบถาม แบบสัมภาษณ์ และแบบบันทึกระดับน้ำตาลในเลือดและความดันโลหิต วิเคราะห์ข้อมูลด้วยสถิติ คือ การแจกแจงความถี่ ร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน ส่วนการเปรียบเทียบผลลัพธ์ก่อนและหลังการทดลองใช้สถิติ t-test และข้อมูลเชิงคุณภาพจากการสัมภาษณ์ใช้การวิเคราะห์เนื้อหา ผลการวิจัยพบว่า กลุ่มตัวอย่างส่วนใหญ่เป็นเพศหญิง อายุ 60 ปีขึ้นไป หลังการทดลองมีคะแนนเฉลี่ยความรู้เพิ่มขึ้นจาก 7.68 (SD=0.96) เป็น 9.58 (SD=0.54) และพฤติกรรมสุขภาพเพิ่มขึ้นจาก 3.19 (SD=0.52) เป็น 4.31 (SD=0.14) อย่างมีนัยสำคัญทางสถิติ (p<.001) ผลลัพธ์ทางคลินิกพบระดับน้ำตาลในเลือดลดลงจาก 130.42 (SD=27.76) mg/dl (มิลลิกรัมต่อเดซิลิตร) เป็น 123.38 (SD=13.56) mg/dl (มิลลิกรัมต่อเดซิลิตร) และความดันโลหิตลดลงจาก 147.66/91.04 (SD=3.20/3.30) mmHg (มิลลิเมตรปรอท) เป็น 139.30/80.57 (SD=1.97/1.33) mmHg (มิลลิเมตรปรอท) ความพึงพอใจต่อรูปแบบในภาพรวมอยู่ในระดับสูง โดยเฉพาะการสนับสนุนจากครอบครัว สรุปได้ว่ารูปแบบพหุวิถีที่บูรณาการวิถีชุมชนและการจัดการตนเองช่วยให้ผู้ป่วยเปลี่ยนพฤติกรรมการดูแลสุขภาพตนเองได้อย่างมีประสิทธิภาพ
