การดูแลผู้ป่วยจิตเภทเรื้อรังด้วยการมีส่วนร่วมของชุมชนต่อ ภาระการดูแลของผู้ดูแลผู้ป่วยจิตเภทเรื้อรังในชุมชน

โรคจิตเภท มีการดำเนินโรคเรื้อรังที่ทำให้ผู้ป่วยสูญเสียบทบาทหน้าที่ในการทำงาน และมีผลกระทบต่อสัมพันธภาพทางสังคมในระยะยาว ภาระการดูแลของผู้ดูแลผู้ป่วยจิตเภทเรื้อรัง เป็นปัญหาที่ส่งผลกระทบต่อทั้งตัวผู้ดูแลและผู้ป่วย งานวิจัยนี้เป็นการวิจัยกึ่งทดลอง มีวัตถุประสงค์เพื่อศึกษาประสิทธิผลของโปรแกรมการดูแลผู้ป่วยจิตเภทเรื้อรังด้วยการมีส่วนร่วมของชุมชนต่อภาระการดูแลของผู้ดูแลผู้ป่วยจิตเภทเรื้อรังในชุมชน และเพื่อเปรียบเทียบภาระการดูแลของผู้ดูแลผู้ป่วยจิตเภทระหว่างกลุ่มได้รับโปรแกรมการดูแลผู้ป่วยจิตเภทเรื้อรังด้วยการมีส่วนร่วมของชุมชน กับกลุ่มที่ได้รับ
การพยาบาลตามปกติ กลุ่มตัวอย่าง คือ ผู้ดูแลหลักในครอบครัวที่ให้การดูแลผู้ป่วยจิตเภทเรื้อรัง จำนวน 20 คน และทีมชุมชน จำนวน 3 คน ซึ่งมีคุณสมบัติครบตามเกณฑ์ โดยกลุ่มทดลองได้รับการดูแลตามโปรแกรมการดูแลผู้ป่วยจิตเภทเรื้อรังด้วยการมีส่วนร่วมของชุมชน ส่วนกลุ่มควบคุมได้รับการดูแลตามปกติ เครื่องมือที่ใช้ในการการวิจัย คือ (1) โปรแกรมการดูแลผู้ป่วยจิตเภทเรื้อรังด้วยการมีส่วนร่วมของชุมชน (2) แบบวัดภาระการดูแลของผู้ดูแลผู้ป่วยจิตเภท ผลการวิจัยที่สำคัญ สรุปได้ดังนี้ (1) ภาระการดูแลของผู้ดูแลผู้ป่วยจิตเภทเรื้อรังในชุมชน ภายหลังได้รับโปรแกรมการดูแลผู้ป่วยจิตเภทเรื้อรัง
ด้วยการมีส่วนร่วมของชุมชน ต่ำกว่าก่อนได้รับโปรแกรม อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (2) ภาระการดูแลของผู้ดูแลผู้ป่วยจิตเภทในชุมชนกลุ่มที่ได้รับโปรแกรมการดูแลผู้ป่วยจิตเภทเรื้อรังด้วยการมีส่วนร่วมของชุมชน ต่ำกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 จากผลการศึกษาในครั้งนี้มีข้อเสนอแนะว่าการดูแลผู้ป่วยจิตเภทเรื้อรังด้วยการมีส่วนร่วมของชุมชน ช่วยลดภาระจากการดูแลของผู้ดูแลผู้ป่วยจิตเภทเรื้อรังในชุมชนลงได้ ดังนั้น จึงเป็นรูปแบบการพยาบาลที่ควรพิจารณาประยุกต์ใช้ในการดูแลผู้ป่วยโรคเรื้อรังอื่น ๆ ต่อไป
