Abstract

The development of the elderly chronic disease management model by community participation in Na-Aor sub-district, Muang district, Loei province purposes for study as follow: 1) situation and problems related to chronic disease management of the elderly in the community 2) factors of success in the development of chronic disease management of the elderly in the community and 3) Guidelines for the development of a management model chronic diseases of the elderly in the community. This research used a participatory research model (Participatory Action Research)  in the development of a chronic disease management model of the elderly in the community. The sample group is a group of elderly who attended a school for the elderly and officials involved in the elderly care system in the community. The study process is defined as consisting of four phases: 1) Situation study phase Problems and needs 2) Phase of studying factors and conditions 3) Phase of conducting research 4) Monitoring and evaluation period Data were collected by means of observation, questionnaires, and in-depth interviews and group discussion.


The results founded that the guidelines for the development of a chronic disease management model for the elderly by the community had a need to provide services in all aspects at the same time consisting of three aspects: 1) promoting knowledge for self-care and family 2) Organize activities for caring for the elderly with chronic diseases by the community / villages 3) Organizing activities for caring for the elderly with chronic diseases by the local government organization And organizing activities for the care of the elderly with chronic diseases by the state health service.
