Effects of Case Management Program for Older Adults with Hypertension and Chronic Renal Failure at a primary care unit

Abstract

          This quasi-experimental study (Two Group Pretest-Posttest Design) aimed to examine the effectiveness of case management program among older adults with hypertension and chronic kidney disease on self-care behaviors and blood pressure level. Participants were aged over 60 years old with hypertension and chronic kidney disease stage 3 and positive proteinuria (1+ and above). There were 30 participants in each intervention and control group in each group by purposive sampling. The study period was eight weeks. The intervention group received case management program and weekly blood pressure measurement. The control group received standard nursing care. The instruments of this study included: 1) case management program 2) self-care behavior questionnaire 3) blood pressure monitor. Pre and post intervention data were collected and analyzed by the descriptive statistic (average, percent, and standard deviation) and Chi-square, Paired t-test and independent t-test.


          The results showed that: 1) The intervention group received higher average self-care behaviors score at the post-intervention period than the pre-intervention period (p< .001). 2) the intervention group, who received the case management program, was lower in the post-intervention than the pre-intervention period with statistical significance (p-value <.001). 3) the intervention group that received the case management program scored higher self-care behaviors average than the control group with statistical significance of p-value < .001. 4) the intervention group that received the case management program scored higher self-care behaviors average than the control group with statistical significance of p-value < .001.


          This study can conclude that a case management program in older adults with hypertension has improved self-care behaviors, blood pressure level, and mitigated the deterioration process of renal function. This could be applied to other hypertension care in order to maximize the benefits of patient care.
