Abstract

Stroke is one of major health problems of older adults, which lead to disability.  Inappropriate management of hypertension is a leading cause of stroke in this population.  The physical and psychological complications of stroke negatively affect older adults’ quality of life.  With the shortage on community based health care providers in Thailand, training and empowering of family caregivers to provide care and monitor stroke risk factors may be a solution.  This intervention can reduce the incidence of stroke among older adults. The authors propose an empowering caregivers model entitled AWARENESS Model.  The aim of this model is to train the caregivers to provide care to older adults with hypertension and to monitor stroke risk factors.  The model consists of nine components: Aware-raise awareness on stroke, Weight reduction-weight control for older adults with hypertension, Add-integrate coping strategies and way to deal with stress, Reducing sodium and fat-reduce high salt and high fat food consumption, Encourage blood pressure control-monitor blood pressure, No alcohol and smoking-maintain no alcohol and smoke, Exercise-promote continuous moderate intensity physical activity, Social support-collaborate with community health networks, and Strengthen inspiration-motivate caregiver to care for older adults with hypertension.
