Development of an Educational Dialysis Catheter Insertion Model for Chronic Kidney Disease Patients

Abstract

This research and innovation research aimed to develop and evaluate the effectiveness of an educational simulation model for dialysis catheter in patients with chronic kidney disease (CKD). The study was conducted in two phases. The first phase focused on problem analysis and innovation development, using the five steps of the Design Thinking approach: empathize, define, ideate, prototype, and test. The participants included dialysis nurses, who provided kidney replacement therapy (KRT) counseling, as well as CKD patients and their family members receiving counseling at the dialysis unit. The effectiveness of the educational innovation was investigated in the second phase, where the sample group consisted of end-stage CKD patients who had not yet made a decision about their RRT modality and came to receive counseling at the hospital. Participants were selected using purposive sampling. Instruments for data collection consisted of general and clinical information questionnaires and a decision-making assessment form for KRT. The experimental tools included a structured KRT counseling dataset and the educational simulation model with dialysis catheter. Data were analyzed using descriptive statistics, including frequency and percentage.

1. The results demonstrated that the educational model was designed as a human upper-body simulator. The posterior view illustrated internal organs relevant to peritoneal dialysis catheter insertion, while the anterior view displayed various catheter exit-site locations.

2. Following the implementation of this educational tool, 88.24% of CKD patients opted for kidney replacement therapy.

The dialysis catheter insertion model represents a nursing innovation designed to enhance patient education during consultations on renal replacement therapy. This educational tool enables patients and their families to better visualize the procedure, thereby reducing fear and anxiety associated with catheter insertion. Moreover, it supports informed decision-making by helping patients and caregivers select the most appropriate renal replacement method based on their individual contexts.
