Comparison of Access to Dental Services and Oral Health Status among Children Aged 0-12 years between Tumbon Health Promoting Hospital with and without Transferred to the Local Administrative Organization in the Northern Region of Thailand

Abstract

This mixed-methods research aimed to document the access to dental services in children who are 0-12 years of age in Northern Thailand.  Goal was to compare accessibility and oral health status among children aged 0-12 years between the Tambon Health Promoting Hospitals (THPH) that are newly administrated by the Local Administrative Organizations (LGO, and those that are not yet.   Research was divided in 2 steps. The first step was to describe the situation and service model for dental services provided to children aged 0-12 years. A total of 12 THPHs were used for purposive sampling. As an instrument, semi-structural questions were used for conducting in-depth interviews, which were then analyzed with content analysis. The second step was to compare the two variables (accessibility and oral health status) among children aged 0-12 years. The official data on children's access to dental services and their oral health status was analyzed for the period between 2017- 2022, and then rated with both descriptive statistics and chi-square test. The results revealed as follows.

1. The situation on management model and dental service were similar between transferred and non-transferred THPHs, but there were different in details of work certification among dental nurses and dental resources support from CUP.

2. The access to dental services in oral examination in children aged 0-12 years, fluoride application in children aged 0-5 years, and dental sealant on permanent teeth in children aged 6 years were statistically different between those two types of the THPH. However, the oral health status such as tooth decay prevalence in children aged 3, 6, and 12 years were not statistically different.

There should be a certification guideline detailing the work of dental nurses and clear monitoring of indicators in the same direction as well as supporting resources to provide services with fairness and no disparity between the transferred and non-transferred THPHs.
