The Development and Effects of a Clinical Nursing Practice Guideline for Heroine Patients Treated by Methadone Maintenance on Quality of Life

This research and development aimed to develop and evaluate a clinical nursing practice guideline (CNPG) for heroine patients treated by methadone on their quality of life. The development process was divided in two stages. First stage included a literature review, evidence-based searching, holding focus groups with multi-disciplinary teams, and the CNPG development. Content validity was verified by five experts, yielding a value of .98. The quality of the CNPG was examined by four experts. Second stage consisted to try out and test feasibility of CNPG. Participants were 10 registered nurses and 10 heroine patients treated by methadone maintenance. Instrument for data collection were the CNPG feasibility assessment and a questionnaire on satisfaction of nurses. Those were verified by five experts, yielding values of .90 and 1.0, respectively. Questionnaire on quality of life was analyzed using Cronbach's alpha coefficient, yielding a value of .79. Data were analyzed by Wilcoxon Signed Rank test statistic. Results were as follows.
1. The clinical nursing practice guideline (CNPG) for heroine patients treated by methadone maintenance consisted of: title, objectives, target group, flow chart of the CNPG, and the essential contents of the CNPG based on nursing process, including three phases: 1) the preparing nursing phase, 2) the stability nursing phase, and 3) the continuously nursing phase. The overall of AGREE II yielding the value of 92.85.
2. The feasibility of the CNPG by registered nurses. It reported that the possibility using guidelines of the preparing, stability, and continuously nursing phase was 90 -100, 70 -100, and 90 - 100 percentage respectively. The overall satisfaction of using the CNPG was high level (M = 2.90, SD = 0.32). and the median scores of quality of life of the patients after participating the CNPG was statistically significant higher than that before participating the CNPG (z = -2.807, p = .005).
This clinical nursing practice guideline (CNPG) for heroine patients treated by methadone maintenance could be implemented by registered nurses. The nurses could apply the CNPG to care for heroine patients under methadone in clinics and hospitals.
