The Development of a Health Behavior Modification Model to Delay Progression of Chronic Kidney Disease towards the Final Stage of Chronic Kidney Failure among Stage 3 Patients in Tha-sao Subdistrict, Uttaradit Province

This research and development aimed to develop a health behavior modification model to slow down kidney deterioration towards the final stage of chronic kidney failure in stage 3 patients with chronic kidney disease. Research was performed in Tha-sao subdistrict of Uttaradit province. It was divided in four steps: 1) to conduct a situation analysis and health behaviors survey, 2) to develop the health behavior modification model, and test it suitability and feasibility, 3) to implement the model and, 4) the model's evaluation. Participants were 29 stage 3 chronic kidney disease patients, selected by purposive sampling method. The research tool were a questionnaire and an interview guideline. The data were analyzed by descriptive statistics, dependent t-test and content analysis. The results were as follows.
1. The situation analysis and health behavior found that the most of the samples had knowledge regarding stage 3 chronic kidney disease at a moderate level. Self-care behaviors and awareness were at good levels and the level of health literacy was insufficient to comply. It was found that the patients had inappropriate self-care behaviors such as taking painkillers, herbal drug, eating salty food, consuming alcoholic beverages, and not taking the medicine as prescribed by the doctor, forgetting to take medication, and missing hospital follow-up appointments.
2. The developed model consisted of a method for: 1) screening patients, 2) implementing changes in their health behaviors, 3) follow-up on the results of changing health behaviors by monitoring their eGFR, creatinine, and urine microalbumin.
3. The results of the trial using the model found that the average knowledge score, self-care behavior and health literacy were statistically significant higher than before the experiment (p<.05). However, mean score on perception of chronic kidney disease stage 3, eGFR, Creatinine and Urine microalbumin before and after the experiment were no difference.
4. The results found that the patients’ satisfaction was at the highest level. They can apply the knowledge gained to their self-care in order to slow down kidney deterioration. The recommendation was about organizing a self-help group in order to exchange experiences on self-care that should be adjusted from once per month to every two months.
Related health agencies should adopt the model and may adjust the number of time of exchanging experiences that suits the patient group to enable patients to change their health care behaviors.
