Development of a Clinical Nursing Practice Guideline for Patients with ST Elevation Myocardial Infarction (STEMI) at Emergency Department of Yala Hospital

The purpose of this research was to develop and to evaluate a clinical nursing practice guideline (CNPG) for ST-supraventricular myocardial infarction patients admitted in emergency department at Yala Hospital. The study had three steps. The first step consisted in studying the different clinical situations. The second step was to develop and implement new clinical nursing practice guideline. Third step was an assessment of the clinical nursing practice guideline. Research sample consisted of 30 professional nurses and 30 ST lift myocardial infarction patients. The research tools were: 1) a clinical nursing practice guideline (CNPG) for ST-supraventricular myocardial infarction patients, 2) patients' medical records, 3) a questionnaire regarding nurse's satisfaction, 4) a knowledge test, and 5) CNPG performance checklist. Data were analyzed by descriptive statistics and Wilcoxon signed-rank test. Results were as follows:
1. There were 3 major key risks identified in emergency response: 1) screening delay, 2) waiting time for fibrinolytic drugs, and 3) delay to coronary artery opening.
2. Content of new clinical nursing practice guideline for patients with myocardial infarction of type ST-lift includes assessment, blood clotting medication, and coronary artery opening.
3. The effectiveness of applying clinical nursing practice guideline for patients with ST raised myocardial infarction found that the patients were screened (M=4.00, SD=1.51) at the time of EKG examination. The fibrinolytic drug time (M=27.63, SD=3.90) to coronary artery opening (M=113.17, SD=42.05) was significantly lower than the threshold, according to the physician's report (M=6.17, SD=2.45) statistical .001, .001, .001, and .001, respectively. The percentage of nurse’s satisfaction was at the highest level with the continued patient care tracking system (M=4.73, SD=0.44). In order to enhance this guideline, it should be used on a continual basis and follow a standardized protocol
