The Village Health Volunteers (VHV) in a Case Management Role for Disabled People in Community

Case management is a collaborative process in a health team effort for patient care, with the purpose to achieve good health. Most of case managers are nurses who are trained for case management program, and work in a hospital. When patients who are in the need to receive continuous care return to their community, the information must be forwarded to caregivers and families. The Village Health Volunteers (VHV) is a participating group in the performance of duties, which is as a representative of the service in promoting a public health straightly. Therefore, VHV are an important part in the mechanism that takes care of the health of people in rural areas, especially people with disabilities who are under the care of family members. The VHV case manager is helpful in caring people with disabilities. Goals can be achieved by increasing the capabilities and performance in various fields, such as: 1) basic knowledge on the rights of persons with disabilities, 2) providing knowledge and skills for health, screening, planning for caring, monitoring, coordinating, and evaluating, 3) financial management, resource usage, and transferring data, 4) psychosocial management, and 5) developing performance.
