The Effects of a Gastrointestinal and Fatigue Symptom Cluster Management Promoting Program on Symptom Experiences in Patients with Breast Cancer Receiving Chemotherapy

This quasi-experimental study aimed to investigate the effects of a gastrointestinal and fatigue symptom cluster management (GFSCM) promoting program on symptom experiences in patients with breast cancer receiving chemotherapy, at the chemotherapy unit of a hospital center in Southern Thailand. The Symptom Management Model was used as a conceptual framework. A total of 48 participants participated. The first 24 participants were assigned to a control group and received regular care, while the other 24 participants were assigned to an experimental group and received the 21-Day GFSCM promoting program. The GFSCM promoting program consists of three phases: 1) evaluation of symptom experiences of gastrointestinal and fatigue symptom cluster, 2) management of symptoms for gastrointestinal and fatigue symptom cluster, and 3) evaluation and management of symptoms for gastrointestinal and fatigue symptom cluster, in terms of frequency and severity. Contents of GFSCM promoting program and Experience of gastrointestinal and fatigue symptoms clusters were validated by three experts. Reliability of the Experience of gastrointestinal and fatigue symptoms cluster questionnaire yielded a test-retest correlation coefficient of .98 and a content validity for scale of 1.0. Data were analyzed using descriptive statistics, and hypotheses testing were tested using independent t-test and analysis of covariance (ANCOVA).
Results showed that the mean scores for the frequency and severity of gastrointestinal and fatigue symptom cluster in the experience group after participating in the program were significantly lower than those in the control group (p<.001 and p=.001, respectively).
Therefore, nurses are recommended to apply this program in caring for those patients to reduce their gastrointestinal and fatigue symptom cluster.
