Effects of a Clinical Nursing Practice Guideline for Discharge Planning on the Quality of Life of Elderly Stroke Survivors

Elderly patients who survived after a stroke are often left with disabilities, and poor quality of life. Consequently, discharge planning and post-stroke follow-up may enhance their quality of life. This quasi-experimental research aimed to examine the effectiveness of a clinical nursing practice guideline for discharge planning on quality of life of elderly stroke survivors. Thirty elderly patients, diagnosed as their first stroke, were equally assigned into experimental and control groups. Control group received usual nursing discharge plan, whereas the experimental group individually received the 8-week clinical nursing practice guideline of discharge plan, which was based on a clinical guideline development concept from the Thai National Health and Medical Research Council (1999). McKeehan & Coulton’s System Model Instruments were used in this study, including the Demographic Data Interview Form, the Stroke-Specific Quality of Life Scale, yielding a Cronbach’s alpha coefficient reliability of .96. Clinical Nursing Practice Guideline of Discharge Planning for Elderly Stroke Survivor, and The Manual of Self-care Practice and Caring for Elderly Stroke Survivors, had a content validity index of 1. Frequency, percentage, mean, standard deviation, and t-test were used for data analysis.
Results revealed that after receiving the clinical nursing practice guideline for discharge planning, mean score of quality of life in the experimental group was significantly higher (t14=14.50, p<.001) than before receiving (t28=3.63, p<.001) the guideline. Findings suggest that nurses should apply this clinical nursing practice guideline with elderly stroke survivors in order to enhance their quality of life.
