การเปรียบเทียบการเข้าถึงบริการทางทันตกรรมและสภาวะสุขภาพช่องปากของเด็กอายุ 0-12 ปี ระหว่างโรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) ถ่ายโอนและไม่ถ่ายโอน ไปยังองค์กรปกครองส่วนท้องถิ่น ในเขตพื้นที่ภาคเหนือของประเทศไทย

การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาสถานการณ์ รูปแบบการจัดบริการ และการให้บริการทางทันตกรรม เพื่อเปรียบเทียบการเข้าถึงบริการทางทันตกรรมและสภาวะสุขภาพช่องปากของเด็กอายุ 0-12 ปี ระหว่างโรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) ถ่ายโอนและไม่ถ่ายโอนไปยังองค์กรปกครองส่วนท้องถิ่น (อปท.) ในเขตพื้นที่ภาคเหนือของประเทศไทย ประกอบด้วย 2 ขั้นตอน ขั้นตอนที่ 1 ศึกษาสถานการณ์ รูปแบบการบริหารจัดการ และการให้บริการทางทันตกรรม คัดเลือก รพ.สต. ที่มีการจัดบริการทางทันตกรรมร่วมกันระหว่างคู่สัญญาบริการสาธารณสุขระดับปฐมภูมิ (CUP) ภายใต้สังกัดกระทรวงสาธารณสุขแบบเฉพาะเจาะจง จำนวน 12 แห่ง สัมภาษณ์เชิงลึกในผู้รับผิดชอบงานทันตกรรม โดยใช้แบบสัมภาษณ์กึ่งโครงสร้าง วิเคราะห์เนื้อหา และขั้นตอนที่ 2 เปรียบเทียบการเข้าถึงบริการทางทันตกรรมและสภาวะสุขภาพช่องปาก โดยใช้ข้อมูลการได้รับบริการทางทันตกรรมและการตรวจสุขภาพช่องปาก จากฐานข้อมูลของระบบกระทรวงสาธารณสุข ระหว่างปีงบประมาณ 2560 - 2565 วิเคราะห์และนำเสนอข้อมูลด้วยสถิติเชิงพรรณนา และ สถิติ Chi-square Test
1. สถานการณ์ รูปแบบการจัดบริการ และการให้บริการทางทันตกรรม โดยส่วนใหญ่เป็นไปในทิศทางเดียวกันทั้ง รพ.สต. ถ่ายโอนและไม่ถ่ายโอน แตกต่างกันที่รายละเอียดของการรับรองการทำงานของทันตาภิบาล และการสนับสนุนอุปกรณ์เครื่องมือทางทันตกรรม
2. การเข้าถึงบริการทางทันตกรรม ได้แก่ การตรวจสุขภาพช่องปากเด็กอายุ 0-12 ปี การเคลือบหรือทาฟลูออไรด์เฉพาะที่เด็กอายุ 0-5 ปี และการเคลือบหลุมร่องฟันแท้เด็กอายุ 6 ปี แตกต่างกัน อย่างมีนัยสำคัญทางสถิติ สภาวะสุขภาพช่องปาก ได้แก่ การมีฟันผุในกลุ่มเด็กอายุ 3, 6 และ 12 ปี ไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ
ควรมีแนวทางการรับรองที่มีรายละเอียดการทำงานของทันตาภิบาลและการกำกับติดตามตัวชี้วัดที่ชัดเจนเป็นไปในทิศทางเดียวกัน รวมถึงการสนับสนุนทรัพยากรเพื่อการจัดบริการที่มีความเป็นธรรม ไม่ให้เกิดความเหลื่อมล้ำระหว่าง รพ.สต. ถ่ายโอนและไม่ถ่ายโอน
