การพัฒนาและผลการใช้แนวปฏิบัติการพยาบาลผู้ป่วยเฮโรอีน ที่บำบัดรักษาด้วยยาเมทาโดนระยะยาวต่อคุณภาพชีวิต

การวิจัยและพัฒนาครั้งนี้ มีวัตถุประสงค์เพื่อพัฒนาและประเมินผลการใช้แนวปฏิบัติการพยาบาลผู้ป่วยเฮโรอีนที่บำบัดรักษาด้วยยาเมทาโดนระยะยาวต่อคุณภาพชีวิต โดยมีกระบวนการพัฒนาเป็น 2 ระยะ คือ 1) พัฒนาแนวปฏิบัติฯ ประกอบด้วย กําหนดประเด็นปัญหาโดยการสนทนากลุ่มของทีมผู้ปฏิบัติงานคลินิกเมทาโดน ทบทวนวรรณกรรมและหลักฐานเชิงประจักษ์ พัฒนาร่างแนวปฏิบัติฯ ตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน ได้ค่า CVI เท่ากับ .98 และตรวจสอบคุณภาพแนวปฏิบัติฯ โดยผู้ทรงคุณวุฒิ 4 คน และ 2) ทดลองใช้และประเมินผลการใช้แนวปฏิบัติฯ กลุ่มตัวอย่างคือพยาบาลวิชาชีพ 10 คน และผู้ป่วยเฮโรอีน 10 คน เครื่องมือ ได้แก่ แบบประเมินความเป็นไปได้ ความพึงพอใจ ตรวจสอบความตรงเชิงเนื้อหา โดยผู้ทรงคุณวุฒิ 5 คน ได้ค่า CVI เท่ากับ .90 และ 1.0 ตามลำดับ และแบบประเมินคุณภาพชีวิต ได้ค่าความเชื่อมั่น เท่ากับ .79 วิเคราะห์ข้อมูลเชิงพรรณนา และสถิติ Wilcoxon Signed Rank test ผลการวิจัย พบว่า
1. แนวปฏิบัติการพยาบาลผู้ป่วยเฮโรอีนที่บำบัดรักษาด้วยยาเมทาโดนระยะยาว ประกอบด้วย ชื่อเรื่อง วัตถุประสงค์ กลุ่มเป้าหมาย ผังขั้นตอนแนวปฏิบัติฯ และสาระสำคัญของแนวปฏิบัติฯ โดยใช้กระบวนการพยาบาล ประกอบด้วย 3 ระยะ ได้แก่ 1) การพยาบาลระยะเริ่มต้น 2) การพยาบาล ระยะคงที่ และ 3) การพยาบาลระยะต่อเนื่อง ตรวจสอบคุณภาพแนวปฏิบัติฯ ด้วย AGREE II เท่ากับ ร้อยละ 92.85
2. ความเป็นไปได้ของการนำใช้แนวปฏิบัติฯ โดยพยาบาลวิชาชีพ พบว่า ความเป็นไปได้ ในการนำใช้แนวปฏิบัติฯ การพยาบาลระยะเริ่มต้น เท่ากับ ร้อยละ 90 - 100 การพยาบาลระยะคงที่ เท่ากับ ร้อยละ 70 - 100 และการพยาบาลระยะต่อเนื่อง เท่ากับ ร้อยละ 90 - 100 โดยความพึงพอใจของการใช้แนวปฏิบัติฯ ภาพรวมอยู่ในระดับมาก (M = 2.90, SD = 0.32) และค่าคะแนนมัธยฐานคุณภาพชีวิตของผู้ป่วยเฮโรอีนหลังได้รับการดูแลตามแนวปฏิบัติฯ สูงกว่าก่อนได้รับการดูแล ตามแนวปฏิบัติฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .01 (z = -2.807, p = .005)
แนวปฏิบัติฯ นี้สามารถให้พยาบาลวิชาชีพนำไปใช้ในการดูแลผู้ป่วยเฮโรอีนในคลินิกเมทาโดนโรงพยาบาลธัญญารักษ์ และโรงพยาบาลอื่น ๆ
