ความชุกและปัจจัยที่ส่งผลต่อความผิดปกติทางระบบกระดูกและกล้ามเนื้อ ของชาวนา อำเภอวังทอง จังหวัดพิษณุโลก

การวิจัยแบบภาคตัดขวางนี้มีวัตถุประสงค์เพื่อศึกษาความชุกและปัจจัยที่ส่งผลต่อความผิดปกติทางระบบกระดูกและกล้ามเนื้อของเกษตรกรชาวนาอำเภอวังทอง จังหวัดพิษณุโลก จำนวน 176 คน เก็บรวบรวมข้อมูลโดยใช้แบบสัมภาษณ์ที่ประยุกต์จากแบบสอบถามมาตรฐานเกี่ยวกับความผิดปกติทางระบบโครงร่างและกล้ามเนื้อ Standardized Nordic Questionnaire แบบประเมินความเสี่ยงความผิดปกติของระบบกระดูกกระดูกและกล้ามเนื้อ ของกรมควบคุมโรค และแบบสอบถามความเครียดที่เกิดขึ้นจากการทำงาน Karasek’s Job Content Questionnaire ฉบับภาษาไทย โดยแบบสอบถามผ่านการตรวจสอบความตรงทางเนื้อหาและหาค่าสัมประสิทธิ์แอลฟาของครอนบาค เท่ากับ .82 วิเคราะห์ข้อมูลโดยใช้ค่าเฉลี่ย ร้อยละ และสถิติ Binary Logistic Regression Analysis ผลการวิจัยพบว่า
1. ความชุกในช่วง 7 วันที่ผ่านมา กลุ่มตัวอย่างมีความผิดปกติทางระบบกระดูกและกล้ามเนื้อ (ส่วนต่าง ๆ ของร่างกาย 12 ส่วน) มากที่สุด 3 อันดับแรก ได้แก่ หลังส่วนล่าง หัวเข่า และหัวไหล่ คิดเป็นร้อยละ 69.30, 53.40 และ 42 ตามลำดับ นอกจากนี้ผลการวิเคราะห์ ความชุกในช่วง 12 เดือนที่ผ่านมา พบว่ากลุ่มตัวอย่างมีความผิดปกติทางระบบกระดูกและกล้ามเนื้อ (ส่วนต่าง ๆ ของร่างกาย 12 ส่วน) มากที่สุด 3 อันดับแรก ได้แก่ หลังส่วนล่าง หัวเข่า และคอ คิดเป็นร้อยละ 68.20, 45.50 และ 38.60 ตามลำดับ
2. ปัจจัยที่ส่งผลต่อการเกิดความผิดปกติทางระบบกระดูกและกล้ามเนื้ออย่างมีนัยสำคัญทางสถิติที่ระดับความเชื่อมั่น 95% ได้แก่ ปัจจัยด้านการทำงาน สภาพสิ่งแวดล้อมทั่วไปหรือลักษณะท่าทางการทำงานของกลุ่มตัวอย่าง คือ การบิดลำตัวหรือเอี้ยวตัวเสมอๆ และลักษณะการทำนาที่มีการก้มต่อเนื่องขณะทำงานเสมอ อย่างมีนัยสำคัญทางสถิติ (ORadj = 0.02, 95%CI: 0.01-0.64)
ดังนั้นหน่วยงานที่เกี่ยวข้องจึงควรวางแผนเพื่อหาแนวทางในการป้องกันและแก้ไขพฤติกรรม ด้านสุขภาพในการทำงานของเกษตรกร เพื่อลดภาวะการณ์เจ็บป่วยจากการทำงาน รวมถึงการจัดทำโปรแกรมปรับเปลี่ยนพฤติกรรมสุขภาพจากการทำงานในเกษตรกรชาวนา เพื่อลดความเสี่ยงของการเกิดความผิดปกติทางระบบกระดูกและกล้ามเนื้อต่อไป
