การพัฒนาหลักสูตรการอบรมเรื่อง “การปฐมพยาบาลผู้ป่วยฉุกเฉิน” สำหรับชุมชนดอนแก้ว อ. แม่ริม จ. เชียงใหม่

การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วมนี้ มีวัตถุประสงค์เพื่อ 1) ศึกษาปัญหาและความต้องการหลักสูตรอบรมการปฐมพยาบาลผู้ป่วยฉุกเฉินสำหรับชุมชนดอนแก้ว 2) พัฒนาหลักสูตร 3) ประเมินประสิทธิผลหลักสูตร ดำเนินการวิจัย 3 ระยะ ระยะที่ 1 ศึกษาปัญหาและความต้องการหลักสูตรโดยการสนทนากลุ่มกับประชาชนในชุมชน ประกอบด้วยผู้นำและบุคลากรสุขภาพในชุมชน จำนวน 26 คน เลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง เครื่องมือที่ใช้คือประเด็นคำถามในการสนทนากลุ่ม เสนอให้ผู้ทรงคุณวุฒิ 3 คน ตรวจสอบและแก้ไข วิเคราะห์ข้อมูลด้วยวิธีวิเคราะห์เนื้อหา ระยะที่ 2 พัฒนาหลักสูตรโดยนำผลการสนทนากลุ่มในระยะที่ 1 และการทบทวนวรรณกรรมมายกร่างหลักสูตร ผ่านการตรวจสอบจากผู้ทรงคุณวุฒิ 3 คน เพื่อพิจารณาความเหมาะสม ความเป็นไปได้ และประโยชน์ต่อผู้เข้าอบรม ระยะที่ 3 ศึกษาประสิทธิผลของหลักสูตร เลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง ประกอบด้วยอาสาสมัครสาธารณสุขประจำหมู่บ้าน (อสม) และครูผู้มีหน้าที่ปฐมพยาบาลในชุมชน จำนวน 35 คน เครื่องมือที่ใช้ในการวิจัยคือแบบประเมินความรู้และแบบประเมินทักษะการปฐมพยาบาลผู้ป่วยฉุกเฉิน รวมทั้งแบบประเมินความพึงพอใจของผู้เข้ารับการอบรม ตรวจสอบความตรงของเนื้อหาของแบบประเมินทั้ง 3 ฉบับ โดยผู้ทรงคุณวุฒิ 3 คน และวิเคราะห์ความเที่ยงของเนื้อหาในแบบประเมินความรู้และแบบประเมินทักษะ ด้วยสถิติ Kuder - Richardson 20 (KR 20) ได้ค่าความเที่ยงเท่ากับ .76 และ .79 ตามลำดับ ส่วนแบบสอบถามความพึงพอใจ ได้ค่าสัมประสิทธิ์แอลฟ่าของครอนบาคเท่ากับ .80 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และสถิติ Paired t-test ผลวิจัยพบว่า
1. ชุมชนดอนแก้วมีปัญหาผู้ป่วยฉุกเฉินจำนวนมากแต่ผู้ให้ความช่วยเหลือไม่มีความรู้และทักษะในการปฐมพยาบาลที่ถูกต้อง จึงต้องการให้มีการพัฒนาหลักสูตรอบรมการปฐมพยาบาลผู้ป่วยฉุกเฉินสำหรับชุมชน
2. หลักสูตรอบรมการปฐมพยาบาลผู้ป่วยฉุกเฉิน ประกอบด้วยกิจกรรม 1) ทดสอบความรู้เรื่องการปฐมพยาบาลผู้ป่วยฉุกเฉินก่อนการอบรม 2) ให้ความรู้เรื่องการปฐมพยาบาลผู้ป่วยฉุกเฉิน 3) ฝึกทักษะการปฐมพยาบาลผู้ป่วยฉุกเฉิน 4) ทดสอบความรู้เรื่องการปฐมพยาบาลผู้ป่วยฉุกเฉินหลังการอบรมและการให้ข้อเสนอแนะ โดยใช้เวลาทั้งหมด 8 ชั่วโมง
3. ผู้เข้าอบรมมีความรู้ตามหลักสูตรหลังการอบรมมากกว่าก่อนการอบรมอย่างมีนัยสำคัญทางสถิติที่ระดับ .001 และมีทักษะในการปฐมพยาบาลผู้ป่วยฉุกเฉินหลังจากการอบรมคิดเป็นร้อยละ 75.80 ซึ่งสูงกว่าเกณฑ์ร้อยละ 60.00 รวมทั้งผู้เข้าอบรมมีความพึงพอใจต่อหลักสูตรการอบรมในระดับมาก คิดเป็นร้อยละ 85.78
ควรใช้หลักสูตรนี้อบรม อสม ครู และนักเรียนในเขตชุมชนดอนแก้วและชุมชนอื่นๆในอำเภอแม่ริมให้ครบทุกคน และสนับสนุนอุปกรณ์การปฐมพยาบาลให้เพียงพอ เพื่อสามารถให้การปฐมพยาบาลได้อย่างมีประสิทธิภาพ
