อาสาสมัครสาธารณสุขผู้จัดการรายกรณีในการดูแลคนพิการในชุมชน: บทบาทที่ท้าทาย

บทความนี้มีวัตถุประสงค์เพื่อนำเสนอแนวคิดการจัดการรายกรณี บทบาทหน้าที่การจัดการรายกรณี สมรรถนะของ อสม.ผู้จัดการรายกรณี และประสบการณ์การดำเนินการพัฒนาศักยภาพ อสม.ในการดูแลคนพิการในชุมชน ซึ่งการจัดการรายกรณี เป็นกระบวนการประสานความร่วมมือของผู้ที่มีบทบาทเกี่ยวข้องกับการดูแลผู้ป่วย เพื่อให้เกิดผลลัพธ์ด้านสุขภาพที่ดี ผู้จัดการรายกรณีส่วนใหญ่เป็นพยาบาลที่ผ่านการอบรมหลักสูตรการจัดการรายกรณีที่ปฏิบัติงานในโรงพยาบาล เมื่อผู้ป่วยที่ต้องได้รับการดูแลอย่างต่อเนื่องคืนกลับสู่ชุมชน จึงต้องมีการส่งต่อข้อมูลให้ผู้ดูแลและครอบครัว อาสาสมัครสาธารณสุข (อสม.) เป็นผู้มีบทบาทสำคัญในการดูแลและส่งเสริมสุขภาพของคนในชุมชน โดยเฉพาะคนพิการซึ่งอยู่ภายใต้การดูแลของสมาชิกในครอบครัว การส่งเสริมสมรรถนะ อสม.เป็นผู้จัดการรายกรณีในการดูแลคนพิการ สามารถทำได้โดยการเพิ่มพูนขีดความสามารถของ อสม.ให้มีสมรรถนะเพิ่มขึ้นในด้านต่าง ๆ ดังนี้คือ 1) การพิทักษ์สิทธิและให้ความรู้พื้นฐานในสิทธิคนพิการ 2) การให้ความรู้และทักษะในการคัดกรองภาวะสุขภาพ การวางแผนดูแลสุขภาพคนพิการ การติดตามดูแลอย่างต่อเนื่อง การประสานงาน และการประเมินผลลัพธ์ของการดูแล 3) การจัดการด้านการเงิน การใช้ทรัพยากร และการส่งต่อข้อมูล 4) การจัดการด้านจิตสังคม และ5) การพัฒนาการปฏิบัติงาน
