ปัจจัยทำนายการเกิดภาวะขาดออกซิเจนในทารกแรกเกิดในระยะแรกรับ ระยะรอคลอด และระยะคลอด โรงพยาบาลส่งเสริมสุขภาพ ศูนย์อนามัยที่ 1 เชียงใหม่

ภาวะขาดออกซิเจนในทารกแรกเกิดเป็นภาวะแทรกซ้อนที่อาจเกิดขึ้นได้ตลอดกระบวนการคลอด ตั้งแต่ระยะแรกรับ ระยะรอคลอด จนถึงระยะคลอด และเป็นสาเหตุสำคัญของการเสียชีวิตในทารกแรกเกิด การวิจัยเชิงทำนายครั้งนี้ ใช้รูปแบบ retrospective case-control study โดยเก็บข้อมูลจากเวชระเบียนย้อนหลัง ระหว่างปี 2565 – 2568 มีวัตถุประสงค์เพื่อศึกษาปัจจัยทำนายการเกิดภาวะขาดออกซิเจนในทารกแรกเกิดในระยะแรกรับ ระยะรอคลอด และระยะคลอด โรงพยาบาลส่งเสริมสุขภาพ ศูนย์อนามัยที่ 1 เชียงใหม่ กลุ่มตัวอย่าง คือ สตรีตั้งครรภ์ที่มาคลอดบุตร จำนวน 1159 ราย แบ่งเป็นกลุ่มศึกษา 61 ราย และกลุ่มควบคุม 1098 ราย โดยเลือกตามคุณสมบัติที่กำหนดไว้ เครื่องมือที่ใช้ในการวิจัย คือ แบบบันทึกข้อมูลปัจจัยทำนายการเกิดภาวะขาดออกซิเจนในทารกแรกเกิด ที่ผู้วิจัยสร้างขึ้นจากการทบทวนวรรณกรรม วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา สถิติการถดถอยพหุโลจิสติก และพื้นที่ใต้เส้นโค้ง (AuROC)
ผลการวิจัย พบว่าปัจจัยทำนายการเกิดภาวะขาดออกซิเจนในทารกแรกเกิดในระยะแรกรับ ได้แก่ อายุครรภ์
< 34 สัปดาห์ (Adj. OR = 37.60, 95% CI: 10.19-138.77) น้ำคร่ำมีขี้เทาสีเขียวข้น (Adj. OR = 30.71, 95% CI: 9.93-94.91) ทารกท่าขวาง (Adj. OR = 20.78, 95% CI: 1.90-227.40) ภาวะครรภ์เป็นพิษ (Adj. OR = 15.23, 95% CI: 1.33-174.08) ภาวะน้ำคร่ำน้อย (Adj. OR = 9.51, 95% CI: 1.51-59.81) ระดับยอดมดลูก ≥ 38 เซนติเมตร (Adj. OR = 6.80, 95% CI: 2.82-16.41) ภาวะตกเลือดก่อนคลอด (Adj. OR = 5.07, 95% CI: 1.28-20.04) น้ำเดิน ≥ 12 ชั่วโมง (Adj. OR = 4.10, 95% CI: 1.91-8.81) เบาหวานขณะตั้งครรภ์ชนิดควบคุมด้วยอาหาร (Adj. OR = 2.74, 95% CI: 1.23-6.14) และคลอดบุตรครั้งแรก (Adj. OR = 2.27, 95% CI: 1.17-4.42) ปัจจัยทำนายในระยะรอคลอด ได้แก่ อัตราการเต้นของหัวใจทารกในครรภ์ < 110 ครั้ง/นาที (Adj. OR = 7.30, 95% CI: 3.44-15.49) และได้รับยาแก้ปวด (Adj. OR = 3.36, 95% CI: 1.79-6.33) และปัจจัยทำนายในระยะคลอด ได้แก่ ภาวะคลอดติดไหล่ (Adj. OR = 57.14, 95% CI: 14.45-225.91) สายสะดือพันคอแน่น (Adj. OR = 7.92, 95% CI: 1.76-35.68) การคลอดด้วยเครื่องดูดสุญญากาศ (Adj. OR = 5.85, 95% CI: 2.19-15.64) การผ่าตัดคลอด (Adj. OR = 3.22, 95% CI: 1.52-6.82) และคลอดเวรบ่าย (Adj. OR = 2.20, 95% CI: 1.07-4.56) แบบจำลองทำนายมีประสิทธิภาพในการจำแนกการเกิดภาวะขาดออกซิเจนในทารกแรกเกิดในระยะแรกรับ ระยะรอคลอด และระยะคลอด อยู่ในระดับ ดีมาก โดยมีค่า AuROC เท่ากับ 0.862, 0.8056 และ 0.8226 ตามลำดับ
ผลการวิจัยครั้งนี้ สามารถใช้เป็นข้อมูลพื้นฐานในการพัฒนาแนวปฏิบัติการป้องกันการเกิดภาวะขาดออกซิเจนในทารกแรกเกิด เพื่อลดอัตราการเกิดภาวะขาดออกซิเจนในทารกแรกเกิด และพัฒนาคุณภาพการดูแลมารดาและทารกให้มีประสิทธิภาพมากยิ่งขึ้น
