Accuracy of ACR-TIRADS in assessment and diagnosis of thyroid nodule in Sena Hospital, Ayutthaya province

Abstract

The purpose of this research was to determine the diagnostic accuracy of ACR-TIRADS and high malignancy risk ultrasonographic features of thyroid nodules. This retrospective study was collected data on patients who underwent thyroid ultrasonography from 1 April 2014 to 30 April 2020 at Sena hospital. 180 patients were included in this study. The data of personal information, size, ultrasonographic features, ACR-TIRADS, and pathologic result were analyzed by Chi-square test and Fisher's exact test, which statistically significant level at 0.05. Sensitivity, specificity, accuracy, positive predictive value (PPV), and negative predictive value (NPV) of high malignancy risk ultrasonographic features and ACR-TIRADS were calculated. When analyzed the ultrasonographic features of echogenicity, shape, margin, and microcalcification were statistically significant (P=.001). High malignancy risk ultrasonographic features were very hypoechoic, taller than wide shape, irregular margin, and calcification which had high specificity (99.4%, 99.4%, 83.7%, and 88.6%, respectively) and high NPV (95.4%, 94.8%, 97.9%, and 98.0%) Very hypoechoic and taller than wide shape had high PPV (85.7% and 83.3%). ACR-TIRADS had high sensitivity and NPV (92.9% and 99.2%). The ACR-TIRADS is an ultrasound-based risk stratification system that helps screen thyroid cancer and reduces the number of unnecessary biopsies for benign thyroid nodules.
