Abstract

This cross-sectional quantitative study aimed to examine the prevalence of non-utilization of influenza vaccination services and to identify associated factors among older adults aged 70 years and above in Bangkok, Thailand. A total of 430 participants was recruited using a non-probability convenience sampling method. Data were collected through a structured questionnaire that had undergone content validation and reliability assessment. Descriptive statistics and multivariable logistic regression analysis were performed, with statistical significance defined at p < 0.05. The results demonstrated that 12.1% of the participants had not received the influenza vaccine. Significant predictors of vaccine non-uptake included a self-reported history of cancer (aOR = 5.32, 95%CI = 1.33–21.22), absence of information received from healthcare professionals (aOR = 3.93, 95%CI = 1.70–9.07), and a high level of knowledge about influenza and its vaccine (aOR = 3.93, 95%CI = 1.70–9.07).Notably, the association between high knowledge and vaccine refusal suggests that knowledge alone may be insufficient to influence positive vaccination behavior in the absence of trusted and accessible health communication. These findings highlight the critical role of credible information dissemination particularly by healthcare providers in promoting vaccine acceptance among older populations. The study emphasizes the need for targeted interventions that go beyond knowledge provision by incorporating tailored, provider-led communication strategies to address vaccine hesitancy in aging communities.
