บทคัดย่อ

ความเป็นมา: อุทกภัยและดินโคลนถล่มปี พ.ศ. 2567 ในจังหวัดเชียงราย เป็นภัยพิบัติที่มีความรุนแรงระดับวิกฤต ส่งผลกระทบวงกว้างต่อประชาชนและระบบบริการสาธารณสุข จึงมีความจำเป็นต้องถอดบทเรียนเพื่อพัฒนารูปแบบการบริหารจัดการภาวะฉุกเฉินให้มีประสิทธิภาพและสอดคล้องกับบริบทพื้นที่


วัตถุประสงค์: เพื่อศึกษาสถานการณ์และพัฒนารูปแบบการบริหารจัดการภาวะฉุกเฉินด้านการแพทย์และสาธารณสุข กรณีอุทกภัยและดินโคลนถล่ม จังหวัดเชียงราย


วิธีการศึกษา: เป็นการวิจัยและพัฒนา (research and development: R&D) แบ่งเป็น 2 ระยะ ได้แก่ ระยะที่ 1 ศึกษาสถานการณ์และถอดบทเรียนข้อมูลย้อนหลังตามกรอบแนวคิด 2P2R (prevention, preparedness, response, recovery) และระยะที่ 2 พัฒนารูปแบบและประเมินผลความรู้บุคลากร กลุ่มตัวอย่างประกอบด้วยผู้บริหารและผู้ปฏิบัติงาน จำนวน 90 ราย


ผลการศึกษา: การตอบโต้ภาวะฉุกเฉินดำเนินการผ่านศูนย์ปฏิบัติการฉุกเฉิน (Public Health Emergency Operation Center; PHEOC) ร่วมกับระบบ Incident Command System (ICS) สามารถระดมทีมปฏิบัติการ 756 ทีม และให้บริการผู้ประสบภัย 38,180 ราย ครอบคลุมการรักษาพยาบาล การเฝ้าระวังโรค การดูแลสุขภาพจิต และการช่วยเหลือกลุ่มเปราะบาง โดยดำเนินงานตาม 2P2R ครอบคลุมทุกระยะ ปัจจัยความสำเร็จ ได้แก่ โครงสร้างการสั่งการแบบ single command ความเข้มแข็งของเครือข่าย และความพร้อมของทีมปฏิบัติการ ขณะที่ข้อจำกัด ได้แก่ กำลังคนไม่เพียงพอ ข้อมูลกลุ่มเปราะบางไม่เป็นปัจจุบัน ปัญหาการสื่อสารในพื้นที่วิกฤต และข้อจำกัดด้านงบประมาณ ผลการพัฒนารูปแบบเน้นการจัดทำแผนเผชิญเหตุเฉพาะภัย (hazard-specific plans) แผนความต่อเนื่องทางธุรกิจ (business continuity plans) ด้านกำลังคน และการใช้ระบบ Geographic Information System (GIS) ระบุกลุ่มเสี่ยง และการพัฒนาระบบสื่อสารสำรอง ภายหลังการถ่ายทอดรูปแบบ กลุ่มตัวอย่างมีคะแนนความรู้เฉลี่ยร้อยละ 74.5 ± 12.7 ส่วนใหญ่อยู่ในระดับสูงและปานกลาง สะท้อนถึงความเหมาะสมและความเป็นไปได้ในการนำไปใช้จริง


สรุปและข้อเสนอแนะ: การบูรณาการกรอบแนวคิด 2P2R ร่วมกับระบบ ICS ช่วยเพิ่มประสิทธิภาพการจัดการภัยพิบัติ แต่จำเป็นต้องปรับปรุงแผนให้ยืดหยุ่นต่อสถานการณ์วิกฤต พัฒนาศักยภาพบุคลากร และพัฒนาระบบข้อมูลให้เป็นปัจจุบันเพื่อลดความสูญเสียในอนาคต
