อัตราการกลับเป็นซ้ำและปัจจัยที่สัมพันธ์กับการกลับเป็นซ้ำของโรคไทรอยด์เป็นพิษชนิดเกรฟส์ (Graves’ Disease) หลังหยุดยาต้านไทรอยด์

บทคัดย่อ
ความเป็นมา: โรคไทรอยด์เป็นพิษชนิดเกรฟส์ (Graves’ disease) เป็นโรคไทรอยด์เป็นพิษที่พบได้บ่อยที่สุด แนวทางการรักษาในปัจจุบัน แนะนำใช้ยาต้านไทรอยด์เป็นระยะเวลาอย่างน้อย 12-18 เดือน และให้ตรวจระดับ TRAb ก่อนหยุดยา พบการกลับเป็นซ้ำของโรคหลังหยุดยาต้านไทรอยด์แล้วได้บ่อย ร้อยละ 20-50
วัตถุประสงค์: ศึกษาอัตราการกลับเป็นซ้ำและปัจจัยที่สัมพันธ์กับการกลับเป็นซ้ำของโรค Graves’ disease หลังหยุดยาต้านไทรอยด์
วิธีการศึกษา: การศึกษาย้อนหลัง (retrospective cohort study) ด้วยการค้นหาฐานข้อมูลอิเล็กทรอนิกส์ ICD-10 E05.0 เก็บข้อมูลจากเวชระเบียนผู้ป่วย Graves’ disease อายุตั้งแต่ 18 ปีขึ้นไป ที่ได้รับการรักษาในโรงพยาบาลเชียงรายประชานุเคราะห์ ระหว่างเดือนมกราคม พ.ศ. 2558 - ธันวาคม พ.ศ. 2567
ผลการศึกษา: ผู้ป่วย Graves’ disease จำนวน 284 ราย เป็นเพศหญิงร้อยละ 80 อายุเฉลี่ย 40 ปี พบการกลับเป็นซ้ำของโรคหลังหยุดยาต้านไทรอยด์ 121 ราย (ร้อยละ 42.6) ปัจจัยที่สัมพันธ์กับการกลับเป็นซ้ำอย่างมีนัยสำคัญทางสถิติ ได้แก่ อาการแสดงทางตา (adjusted odds ratio (aOR) 2.55; 95% CI: 1.34-4.88; p=0.005) ขนาดต่อมไทรอยด์โต (aOR 2.74; 95% CI: 1.39-5.39; p=0.003) ระดับ FT4 ขณะวินิจฉัย ≥ 2 เท่าของค่าปกติ (aOR 2.07; 95% CI: 1.08-3.97; p=0.028) ได้รับยา methimazole (MMI) มากกว่า 2.5 มิลลิกรัมต่อวันก่อนหยุดยา (aOR 1.79; 95% CI: 1.04-3.13; p=0.037) และระยะเวลาช่วงปรับยามากกว่า 6 เดือน (aOR 2.26; 95% CI: 1.21-4.23; p=0.011)
สรุปและข้อเสนอแนะ: ผู้ป่วย Graves’ disease ที่มีอาการแสดงทางตา ขนาดต่อมไทรอยด์โต ระดับ FT4 ขณะวินิจฉัยสูง ได้รับยา MMI มากกว่า 2.5 มิลลิกรัมต่อวันก่อนหยุดยา และระยะเวลาช่วงปรับยานาน เป็นปัจจัยที่สัมพันธ์กับการกลับเป็นซ้ำ ผู้ป่วยเหล่านี้ควรพิจารณาส่งตรวจระดับ TRAb ก่อนหยุดยา นัดติดตามอาการและค่าฮอร์โมนไทรอยด์โดยเฉพาะในช่วง 2 ปีแรกหลังหยุดยา หรือเลือกให้การรักษาด้วยยาต้านไทรอยด์ขนาดต่ำในระยะยาว เพื่อเพิ่มโอกาสในการหายขาดจากตัวโรค
