ปัจจัยที่มีผลต่อการผ่าตัดคลอดในโรงพยาบาลพะเยา

บทคัดย่อ
ความเป็นมา: อัตราการผ่าตัดคลอดมีแนวโน้มเพิ่มขึ้นทั้งในระดับโลกและประเทศไทย แม้ไม่สัมพันธ์กับการลดลงของอัตราการเสียชีวิตของมารดาและทารก โรงพยาบาลพะเยายังคงมีอัตราการผ่าตัดคลอดสูงขึ้นอย่างต่อเนื่อง
วัตถุประสงค์: ศึกษาปัจจัยที่มีผลต่อการตัดสินใจผ่าตัดคลอด จัดกลุ่มการผ่าตัดคลอดตาม Robson classification และศึกษาลักษณะใน Robson กลุ่ม 1-4 ซึ่งมีแนวโน้มคลอดปกติได้
วิธีการศึกษา: ศึกษาย้อนหลังจากฐานข้อมูลโรงพยาบาลพะเยาตั้งแต่วันที่ 1 ตุลาคม พ.ศ. 2566 –30 เมษายน พ.ศ. 2568 ในหญิงตั้งครรภ์ที่มาคลอดบุตร ใช้สถิติเชิงพรรณนาและวิเคราะห์ปัจจัยที่มีผลต่อการตัดสินใจผ่าตัดคลอดด้วย multivariable logistic regression
ผลการศึกษา: หญิงตั้งครรภ์ที่มาคลอดบุตรจํานวน 562 ราย พบปัจจัยที่สัมพันธ์กับการผ่าตัดคลอดอย่างมีนัยสําคัญ ได้แก่ ความดันโลหิตสูงเรื้อรัง (aOR 53.09, 95% CI 2.33–1209.02, p=0.013) ไม่เคยมีบุตร (aOR 13.62, 95% CI 1.19–154.84, p=0.035) การคลอดในเวรเช้า (aOR 6.51, 95% CI 2.67–15.87, p<0.001) การคลอดในเวรบ่าย (aOR 2.81, 95% CI 1.05–7.48, p=0.038) และการฝากครรภ์ที่คลินิก (aOR 3.7, 95%CI 1.83–7.47, p<0.001) เมื่อจําแนกตาม Robson classification พบกลุ่ม 5 ร้อยละ 29.7 รองลงมาคือกลุ่ม 2 ร้อยละ 21.2 และกลุ่ม 1 ร้อยละ 15.9 นอกจากนี้ ลักษณะที่พบส่วนใหญ่ในกลุ่ม Robson 1–4 ได้แก่ สัญชาติไทย อายุ 20–34 ปี สิทธิการรักษาหลักประกันสุขภาพถ้วนหน้า ส่วนสูง 145–160 เซนติเมตร น้ําหนักทารก 2,500–3,500 กรัม การคลอดในเวรเช้า และการฝากครรภ์ที่คลินิก
สรุปผลและข้อเสนอแนะ: ปัจจัยที่มีผลต่อการตัดสินใจผ่าตัดคลอด ได้แก่ ความดันโลหิตสูงเรื้อรัง ไม่เคยมีบุตร การคลอดในเวรเช้า และการฝากครรภ์ที่คลินิก โดย Robson classification กลุ่ม 5 มีอัตราการผ่าตัดคลอดสูงสุดและลักษณะส่วนใหญ่ที่พบในกลุ่ม 1–4 ซึ่งเป็นกลุ่มที่มีโอกาสคลอดทางช่องคลอดได้เช่น สัญชาติไทย อายุ 20-34 ปี การคลอดในเวรเช้า และฝากครรภ์ที่คลินิก ดังนั้นควรส่งเสริมการลดอัตราการผ่าตัดคลอดที่ไม่จําเป็นผ่านการให้ความรู้และสนับสนุนการฝากครรภ์อย่างสม่ําเสมอ
