Prevalence of cardiovascular events in patients with hypertensive urgency at Primary Care Unit, Chiangrai Prachanokroh Hospital

Abstract

BACKGROUND: Hypertensive urgency is a condition characterized by critically elevated blood pressure without evidence of acute hypertension-mediated target organ damage. However, failure to achieve blood pressure control may increase the risk of cardiovascular complications, which can lead to disability and mortality. Nevertheless, evidence regarding outcomes in this patient population remains limited.


OBJECTIVE: To determine the prevalence of cardiovascular events within three years after the diagnosis of hypertensive urgency and to identify factors associated with the occurrence of these events.


METHODS: A retrospective cohort study was conducted using medical records from Chiangrai Prachanukroh Hospital between January 1, 2019, and December 31, 2021. The study included 900 patients diagnosed with hypertensive urgency who received care at the Primary Care Unit. The prevalence of cardiovascular events within three years after diagnosis was assessed. Factors associated with the occurrence of cardiovascular events were analyzed using Cox regression analysis. Statistical significance was set at p<0.05.


RESULTS: Among 900 patients, 130 (14.4%) developed cardiovascular complications. The highest incidence occurred during the first year after diagnosis (6.2%), with a decreasing trend over the follow-up period. Factors significantly associated with an increased risk of complications included a history of atrial fibrillation (hazard ratio; HR=6.00), prior stroke (HR=3.06), chronic kidney disease stage ≥4 (HR=2.61), smoking (HR=2.27), and proteinuria ≥1+ (HR=1.60).


CONCLUSIONS AND RECOMMENDATIONS: Among patients with hypertensive urgency, the highest rate of cardiovascular complications occurred during the first year after diagnosis. Therefore, close follow-up during this period, particularly in the first year, is essential. Emphasis should be placed on controlling risk factors and prompt medical evaluation when abnormal symptoms occur.
