บทคัดย่อ
ความเป็นมา: ผู้ป่วยหนักต้องได้รับการติดตามอาการอย่างใกล้ชิดในหอผู้ป่วยวิกฤต (intensive care unit; ICU) มักมีปัญหาทางการแพทย์ที่ซับซ้อน รวมถึงมีอัตราการเสียชีวิตสูง การดูแลผู้ป่วยกลุ่มนี้จำเป็นต้องใช้ทรัพยาการทางการแพทย์มาก การเข้าใจปัจจัยที่ส่งผลต่อการเสียชีวิตในหอผู้ป่วยวิกฤติจึงมีความสำคัญ เพื่อให้ทีมสหสาขาวิชาชีพสามารถปรับปรุงแนวทางการดูแลรักษาและจัดสรรทรัพยากรได้อย่างมีประสิทธิภาพ
วัตถุประสงค์: เพื่อศึกษาปัจจัยที่ส่งผลต่อการเสียชีวิตของผู้ป่วยในหอผู้ป่วยวิกฤตทางอายุรกรรม โรงพยาบาลเชียงรายประชานุเคราะห์
วิธีการศึกษา: การศึกษาแบบ retrospective cohort study ในผู้ป่วยอายุตั้งแต่ 18 ปีขึ้นไป ที่เข้ารับการรักษาในหอผู้ป่วยวิกฤตทางอายุรกรรม โรงพยาบาลเชียงรายประชานุเคราะห์ นานกว่า 24 ชั่วโมง จำนวน 120 ราย ระหว่างวันที่ 1 มีนาคม พ.ศ. 2568 - 31 สิงหาคม พ.ศ. 2568 รวบรวมจากข้อมูลเวชระเบียนอิเล็กทรอนิกส์ ได้แก่ ข้อมูลพื้นฐาน โรคประจำตัว คะแนนความรุนแรงของโรค (SOFA score) และผลตรวจทางห้องปฏิบัติการ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและการวิเคราะห์การถดถอยปัวซอง เพื่อศึกษาปัจจัยที่ส่งผลต่อการเสียชีวิต
ผลการศึกษา: อัตราการเสียชีวิตใน ICU คิดเป็นร้อยละ 23.3 (28 ราย) ผลการวิเคราะห์ปัจจัยเดี่ยวพบว่า กลุ่มที่เสียชีวิตมีคะแนน SOFA score สูงกว่า ค่า S/F ratio ต่ำกว่า ระยะเวลาการใช้เครื่องช่วยหายใจนานกว่า และมีภาวะแทรกซ้อนจากการติดเชื้อในโรงพยาบาลและปอดอักเสบสูงกว่า รวมถึงมีการใช้ยากระตุ้นความดันโลหิต ยาระงับประสาท และยาหย่อนกล้ามเนื้อมากกว่ากลุ่มที่รอดชีวิตอย่างมีนัยสำคัญทางสถิติ (p<0.05) เมื่อวิเคราะห์พหุตัวแปร พบปัจจัยพยากรณ์อิสระที่สัมพันธ์กับการเสียชีวิตอย่างมีนัยสำคัญทางสถิติ 2 ปัจจัย ได้แก่ ค่า S/F ratio ที่ต่ำ (aRR 0.99; 95% CI 0.99–1.00; p=0.029) และการได้รับยาหย่อนกล้ามเนื้อ (aRR 1.84; 95% CI 1.01–3.35; p=0.045)
สรุปและข้อเสนอแนะ: ค่า S/F ratio ที่ต่ำและการใช้ยาหย่อนกล้ามเนื้อเป็นปัจจัยพยากรณ์อิสระที่สำคัญต่อการเสียชีวิตใน ICU การนำค่า S/F ratio มาใช้เพื่อเฝ้าระวังและติดตามอาการทางคลินิก และการทบทวนแนวทางการใช้ยาหย่อนกล้ามเนื้อให้มีความเฉพาะเจาะจงมากขึ้น จะช่วยเพิ่มโอกาสการรอดชีวิตของผู้ป่วยได้ในอนาคต
