The Effects of Nursing Practice Guidelines toward Promoting Medication Adherence for Patients with Pulmonary Tuberculosis Who are Followed up at Tuberculosis Clinic of Chomthong Hospital Chiangmai Province

Abstract
BACKGROUND: Implementing a nursing practice guideline to promote continuous medication adherence among patients with tuberculosis enables elderly patients to receive care appropriate to their conditions, gain knowledge of self-care, and maintain continuous medication adherence, thereby leading to better symptom control, health, and quality of life.
OBJECTIVE: To investigate tuberculosis knowledge, medication adherence levels, and health belief among elderly patients with tuberculosis before and after receiving nursing care based on the nursing practice guideline for promoting continuous medication adherence at the Tuberculosis Clinic, Chomthong Hospital.
METHODS: A quasi-experimental study using a one-group pretest–posttest design without a comparison group was conducted between January 1 and September 30, 2024. Tuberculosis knowledge and medication adherence among elderly patients with tuberculosis were evaluated before and after the intervention. Statistical significance was set at p<0.05.
RESULTS: Before receiving nursing care based on the nursing practice guideline, the mean tuberculosis knowledge score was at a moderate level (8.9 ± 2.2), which increased to a high level after receiving
the guideline (12.1 ± 2.3). The mean medication adherence score remained at a moderate level before and after the intervention (6.1 ± 1.9 and 7.0 ± 0.3, respectively), although the mean score increased following the intervention. Comparison of outcomes before and after implementation of the nursing practice guideline showed that the median tuberculosis knowledge score increased significantly from 9 (IQR 4) to 12 (IQR 1) (p<0.001). Likewise, the median medication adherence score increased significantly from 6 (IQR 3) to 7
(IQR 0) (p=0.048).
CONCLUSIONS AND RECOMMENDATIONS: The nursing practice guideline for promoting continuous medication adherence among patients with tuberculosis should be integrated into routine nursing care for elderly patients to enhance the effectiveness of self-care management among patients with tuberculosis.
