บทคัดย่อ
ความเป็นมา: ภาวะความดันโลหิตสูงวิกฤตชนิดเร่งด่วน เป็นภาวะที่มีระดับความดันโลหิตสูงในเกณฑ์วิกฤตโดยยังไม่พบความเสียหายเฉียบพลันต่ออวัยวะสำคัญ แต่หากไม่สามารถควบคุมความดันโลหิตได้ตามเป้าหมาย อาจเพิ่มความเสี่ยงต่อการเกิดภาวะแทรกซ้อนทางโรคหัวใจและหลอดเลือด ซึ่งนำไปสู่ภาวะทุพลภาพและเสียชีวิตก่อนวัยอันควร อย่างไรก็ตาม ข้อมูลการศึกษาในผู้ป่วยกลุ่มนี้ยังมีจำกัด
วัตถุประสงค์: เพื่อศึกษาความชุกของการเกิดภาวะแทรกซ้อนทางโรคหัวใจและหลอดเลือดภายใน 3 ปีหลังการวินิจฉัยความดันโลหิตสูงวิกฤติชนิดเร่งด่วน และศึกษาปัจจัยที่มีความสัมพันธ์กับการเกิดภาวะแทรกซ้อนดังกล่าว
วิธีการศึกษา: การศึกษาแบบ retrospective cohort study เก็บข้อมูลจากเวชระเบียนผู้ป่วยโรงพยาบาลเชียงรายประชานุเคราะห์ ระหว่างวันที่ 1 มกราคม พ.ศ. 2562 ถึง 31 ธันวาคม พ.ศ. 2564 กลุ่มตัวอย่างคือผู้ป่วยโรคความดันโลหิตสูงวิกฤติชนิดเร่งด่วนที่เข้ารับบริการ ณ หน่วยบริการปฐมภูมิ จำนวน 900 ราย เก็บข้อมูลความชุกของการเกิดภาวะแทรกซ้อนทางโรคหัวใจและหลอดเลือดภายใน 3 ปีหลังการวินิจฉัย วิเคราะห์ปัจจัยที่มีความสัมพันธ์โดยใช้สถิติ Cox regression กำหนดระดับนัยสำคัญทางสถิติที่ p<0.05
ผลการศึกษา: จากจำนวนผู้ป่วย 900 คน พบผู้ป่วย 130 ราย (ร้อยละ 14.4) เกิดภาวะแทรกซ้อนทางโรคหัวใจและหลอดเลือด ซึ่งพบภาวะแทรกซ้อนมากที่สุดในปีแรกหลังการวินิจฉัย (ร้อยละ 6.2) และมีแนวโน้มลดลงตามระยะเวลาการติดตาม ปัจจัยที่มีความสัมพันธ์กับการเพิ่มโอกาสเกิดภาวะแทรกซ้อนอย่างมีนัยสำคัญ ได้แก่ ประวัติโรคหัวใจห้องบนสั่นพลิ้ว (hazard ratio; HR=6.00) ประวัติโรคหลอดเลือดสมอง (HR=3.06) โรคไตเรื้อรังตั้งแต่ระยะที่ 4 ขึ้นไป (HR=2.61) การสูบบุหรี่ (HR=2.27) และการพบโปรตีนในปัสสาวะตั้งแต่ 1+ ขึ้นไป (HR=1.60)
สรุปและข้อเสนอแนะ: ผู้ป่วยความดันโลหิตสูงวิกฤติชนิดเร่งด่วนเมื่อติดตามไป 3 ปี พบภาวะแทรกซ้อนทางโรคหัวใจและหลอดเลือดมากที่สุดในปีแรกหลังการวินิจฉัย ดังนั้นควรให้ความสำคัญกับการติดตามอย่างใกล้ชิด โดยเฉพาะในปีแรก โดยให้คำแนะนำเพื่อควบคุมปัจจัยเสี่ยงและเมื่อมีอาการผิดปกติให้เข้ารับการรักษาอย่างทันท่วงที
