การบริหารจัดการตอบโต้ภาวะฉุกเฉินทางด้านการแพทย์และสาธารณสุข กรณีการแพร่ระบาด ของโรคติดเชื้อไวรัสโคโรนา-2019 จังหวัดเชียงราย

บทคัดย่อ
ความเป็นมา: ภาวะฉุกเฉินทางด้านการแพทย์และสาธารณสุขเกิดขึ้นบ่อยครั้งและมีผลกระทบที่รุนแรงเช่น สถานการณ์การระบาดของโรคติดเชื้อไวรัสโคโรนา-2019 ที่มีผู้ป่วยและเสียชีวิตเป็นจำนวนมาก กระทรวงสาธารณสุขได้นำระบบบัญชาการเหตุการณ์มาประยุกต์ใช้ในการบริหารจัดการภัยพิบัติทางด้านการแพทย์และสาธารณสุข และมีนโยบายกระจายการดำเนินงานสู่ภูมิภาคในระดับจังหวัด แต่การดำเนินงานดังกล่าวต้องมีการปรับใช้ตามสถานการณ์ ศักยภาพองค์กร และบริบทในพื้นที่
วัตถุประสงค์: 1. เพื่อศึกษารูปแบบการบริหารจัดการตอบโต้ภาวะฉุกเฉินทางด้านการแพทย์และสาธารณสุข กรณีการแพร่ระบาดของโรคติดเชื้อไวรัสโคโรนา-2019 จังหวัดเชียงราย 2. เพื่อพัฒนาแนวทางการดำเนินงานในการบริหารจัดการตอบโต้ภาวะฉุกเฉินทางด้านการแพทย์และสาธารณสุข จังหวัดเชียงราย นำไปประยุกต์ใช้กับเหตุการณ์ภัยพิบัติอื่นๆ
วิธีการศึกษา: เป็นการวิจัยแบบผสมผสาน ในรูปแบบเกิดพร้อมกัน ศึกษาข้อมูลย้อนหลังจากจากเอกสาร รายงานการถอดบทเรียน รายงานการประชุม การสัมภาษณ์เชิงลึก การสนทนากลุ่ม และแบบสอบถาม วิเคราะห์ข้อมูลเชิงคุณภาพ โดยการวิเคราะห์เนื้อหา วิเคราะห์ข้อมูลเชิงปริมาณด้วยสถิติเชิงพรรณนา ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน
ผลการศึกษา: จังหวัดเชียงราย ดำเนินการเตรียมความพร้อมในการรับสถานการณ์การระบาดของ
โรคติดเชื้อไวรัสโคโรนา-2019 วางระบบป้องกันโรคติดเชื้อไวรัสโคโรนา-2019 โดยระบบบัญชาการเหตุการณ์ เพื่อบัญชาการและประสานระหว่างหน่วยงาน ด้วยหลัก 2P2R ได้แก่ การดำเนินงานป้องกันและลดผลกระทบ การเตรียมความพร้อม การตอบโต้ และการฟื้นฟู โดยอาศัยกลไกคณะกรรมการโรคติดต่อจังหวัดเชียงราย บูรณาการการดำเนินงานในระดับจังหวัด อำเภอ ตำบล และชุมชน การประเมินผลการดำเนินงานบริหารจัดการตอบโต้ภาวะฉุกเฉินทางด้านการแพทย์และสาธารณสุข กรณีการแพร่ระบาดของโรคติดเชื้อไวรัสโคโรนา-2019 จังหวัดเชียงราย พบว่าอยู่ในระดับสูง (Mean=2.49, SD=0.63) แนวทางการพัฒนาการบริหารจัดการตอบโต้ภาวะฉุกเฉินทางด้านการแพทย์และสาธารณสุข ในจังหวัดเชียงราย ได้มีการปรับโครงสร้างการบัญชาการเหตุการณ์ให้เข้ากับสภาพปัญหาในพื้นที่และศักยภาพขององค์กร นอกจากนี้ยังได้ปรับปรุงแผนปฏิบัติและมาตรฐานการปฏิบัติงานให้สอดคล้องกับการปฏิบัติจริง โดยใช้กระบวนการมีส่วนร่วมจากบุคลากรทุกระดับและทุกภาคส่วน
สรุปและข้อเสนอแนะ: ก่อนการแพร่ระบาดของโรคติดเชื้อไวรัสโคโรนา-2019 จังหวัดเชียงรายไม่มีแผนเตรียมรับมือภัยพิบัติตามหลัก 2P2R และบทบาทผู้รับผิดชอบไม่ชัดเจน เมื่อเกิดเหตุการณ์พบปัจจัยความสำเร็จในด้านการบัญชาการเหตุการณ์ และความร่วมมือของทุกภาคส่วน แต่พบอุปสรรคในการวางแผนจัดการโรคอุบัติใหม่ ที่เกี่ยวข้องกับบริบทพื้นที่และความหลากหลายของชนชาติพันธุ์ หลังเกิดเหตุการณ์สำนักงานสาธารณสุขจังหวัดเชียงรายจึงมีการเตรียมความพร้อมปรับปรุงแผนต่างๆ และปรับโครงสร้างการบัญชาการเหตุการณ์โดยกระบวนการมีส่วนร่วม ข้อเสนอแนะควรมีการทบทวนแผน และบทบาทหน้าที่ตามผังโครงสร้างการบัญชาการเหตุการณ์ทุกปี
