ผลของโปรแกรมการสร้างเสริมการรับรู้ความสามารถตนเองต่อพฤติกรรมสุขภาพ ของผู้สูงอายุโรคความดันโลหิตสูงที่ควบคุมระดับความดันโลหิตไม่ได้

บทนำ: ความดันโลหิตสูงในผู้สูงอายุที่ควบคุมไม่ได้เป็นปัจจัยเสี่ยงทำให้เกิดโรคหัวใจและหลอดเลือด การจัดการความดันโลหิตสูงอย่างมีประสิทธิผลจึงมีความสำคัญอย่างยิ่ง
วัตถุประสงค์การวิจัย: ศึกษาผลของโปรแกรมการสร้างเสริมการรับรู้ความสามารถตนเองต่อพฤติกรรมสุขภาพของผู้สูงอายุโรคความดันโลหิตสูงที่ควบคุมระดับความดันโลหิตไม่ได้
ระเบียบวิธีวิจัย: เป็นการวิจัยกึ่งทดลอง แบบสองกลุ่ม วัดก่อนและหลังการทดลอง กลุ่มตัวอย่างเป็นผู้ป่วยโรคความดันโลหิตสูง คัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง จำนวน 56 คน เป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 28 คน เครื่องมือวิจัย คือ 1) เครื่องมือคัดกรอง คือ MMSE-Thai 2002 และแบบประเมินความเครียด 2) เครื่องมือเก็บข้อมูล คือ แบบสัมภาษณ์ข้อมูลทั่วไป พฤติกรรมการใช้ยา การควบคุมอาหารและการออกกำลังกาย 3) เครื่องมือทดลอง คือ โปรแกรมการสร้างเสริมการรับรู้ความสามารถตนเอง มีระยะเวลา 12 สัปดาห์ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา Paired-sample t-tests และ Independent-sample t-tests
ผลการวิจัย: หลังเข้าร่วมโปรแกรมการสร้างเสริมการรับรู้ความสามารถตนเอง กลุ่มทดลองมีค่าเฉลี่ยความดันโลหิตลดลงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p < .001) แต่พบว่า การรับรู้ความสามารถต่อพฤติกรรมสุขภาพด้านการรับประทานอาหาร การรับประทานยา และการออกกำลังกายหลังเข้าร่วมโปรแกรมกลุ่มทดลองและกลุ่มควบคุม ไม่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p >.05)
สรุปผล: โปรแกรมฯ นี้ไม่สามารถช่วยปรับเปลี่ยนพฤติกรรมสุขภาพได้ แต่สามารถช่วยลดระดับความดันโลหิตของผู้สูงอายุโรคความดันโลหิตสูงที่ควบคุมระดับความดันโลหิตไม่ได้
ข้อเสนอแนะ: แม้การศึกษานี้จะไม่พบความแตกต่างในด้านการปรับเปลี่ยนพฤติกรรมสุขภาพ แต่สามารถใช้เป็นแนวทางในการลดระดับความดันโลหิตสำหรับผู้สูงอายุที่เป็นโรคความดันโลหิตสูงที่ไม่สามารถควบคุมระดับความดันโลหิตได้
