Factors related to foot ulcers of type 2 diabetic patients in diabetes clinics

Introduction: Diabetic foot ulcer in diabetic patients with uncontrolled glucose levels is a major complication affecting the risk of infection, leading to foot amputation and finally affecting the quality of life of the patients and relatives
Objective: To study factors related to foot ulcers in type 2 diabetic patients with uncontrolled glucose in the diabetes clinic.
Research methodology: The research design was a cross-sectional survey study. Three hundred fifty type 2 diabetic patients aged between 40-70 years with an HbA1C level of 8% or higher were simply random sampled into the study at Diabetes clinic, Phra Samut Chedi Sawatyanon Hospital. Research tools included: 1) Personal information; 2) Foot care behaviors and 3) Assessing the risk of foot ulcers by looking at the feet, checking the pulse, and testing the sensation of the feet. Data were analyzed by descriptive statistics. Factors related to the incidence of foot ulcers were analyzed by using Chi-square statistics.
Results: The incidence of diabetic foot ulcers in diabetic patients with uncontrolled glucose levels was correlated with loss of sensation in the feet, hard skin, and cracked heels (p<0.01); followed by body mass index, smoking, decreased blood circulation in the feet, deformed feet, and foot care behaviors (p<0.5).
Conclusion: The incidence of diabetic foot ulcers is associated with many factors. Preventing foot ulcers is necessary to reduce foot amputation. However, diabetic patients should take part in taking care of their feet.
Recommendations: The multidisciplinary team responsible for the diabetes clinic should arrange a service to assess the risk of foot ulcers for diabetic patients as well as provide advice on foot care for diabetic patients at risk for foot ulcers.
