The Effectiveness of Implementing Clinical Practice Guidelines for Prevention of Preterm Labor at the Antenatal Care Unit, Nakhonpathom Hospital

Abstract

     Premature labor is the most common obstetric complication required hospital stay land to be treated with medication to prevent contraction and premature labor. If the treatment is unsuccessful, there will be a preterm labor with negative effects to newborns. This study was a
quasi-experimental nonequivalent control groups design. The aim was to examine the effectiveness of implementing clinical practice guidelines (CPGs) for prevention preterm birth in pregnant women at the antenatal care clinic of Nakhonpathom Hospital. The control group comprised of 89 cases that received usual care between February to March 2017. Whereas, the experimental group of 103 cases received antenatal care according to CPGs for prevention preterm birth between February to March 2018. Research instruments were 1) the CPGs for prevention of preterm birth 2) the Personal data recording form, 3) the record for number of cases with term labor, 4) the record for number of cases with term labor after received labor suppressant medication, 5) the dexamethasone medication record form, and 6) the record of cervical dilatation on admission. Data was analyzed using descriptive and chi-square statistics.
      The results revealed that pregnant women in the control group that received CPGs for prevention preterm birth had clinical outcomes related to term labor, term labor after receiving labor suppressant medicine, receiving dexamethasone for 4 doses, and cervical dilation < 3 cms at
admission significantly higher than the control group (p<.05). The results confirm the effectiveness of implementation of CPGs for the prevention of preterm
birth among pregnant women receiving care at antenatal clinic.
