บทคัดย่อ

บทนำ: การเกิดแผลที่เท้าของผู้ป่วยเบาหวานที่ควบคุมระดับน้ำตาลไม่ได้เป็นภาวะแทรกซ้อนที่สำคัญที่ทำให้ผู้ป่วยเสี่ยงต่อการติดเชื้อนำไปสู่การสูญเสียเท้า และส่งผลต่อคุณภาพชีวิตของผู้ป่วยและญาติ

วัตถุประสงค์การวิจัย เพื่อศึกษาปัจจัยที่มีความสัมพันธ์กับการเกิดแผลที่เท้าของผู้ป่วยเบาหวานชนิดที่ 2 ที่ควบคุมระดับน้ำตาลไม่ได้ ในคลินิกเบาหวาน

ระเบียบวิธีวิจัย การวิจัยเชิงสำรวจแบบตัดขวาง กลุ่มตัวอย่างเป็นผู้ป่วยเบาหวาน อายุระหว่าง 40-70 ปี ที่มีระดับ HbA1C ตั้งแต่ 8% ที่มารับบริการที่คลินิกเบาหวาน โรงพยาบาลพระสมุทรเจดีย์ จำนวน 350 คน เลือกด้วยการสุ่มอย่างง่าย เครื่องมือในการวิจัยประกอบด้วย1) ข้อมูลส่วนบุคคล 2)พฤติกรรมการดูแลเท้า และ3)ประเมินเท้าจากแบบประเมินด้วยการดูเท้า คลำชีพจร และทดสอบความรู้สึกที่เท้า วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณา วิเคราะห์ปัจจัยที่มีความสัมพันธ์ต่อการเกิดแผลที่เท้าทดสอบด้วยสถิติไคสแคว์

ผลการวิจัย การเกิดแผลที่เท้าของผู้ป่วยเบาหวานที่ควบคุมระดับน้ำตาลไม่ได้ มีความสัมพันธ์กับปัจัยดังต่อไปนี้ การสูญเสียความรู้สึกที่เท้า เท้ามีหนังหนาแข็ง และส้นเท้าแตก (p<0.01) รองลงมาคือดัชนีมวลกาย การสูบบุหรี่ การไหลเวียนที่เท้าลดลง ลักษณะเท้าผิดรูป และพฤติกรรมการดูแลเท้า (p<0.5)

สรุป การเกิดแผลที่เท้าในผู้ป่วยเบาหวานมีความสัมพันธ์กับหลายปัจจัย การป้องกันก่อนการเกิดแผลที่เท้าเป็นสิ่งที่จำเป็นที่จะช่วยลดปัญหาการถูกตัดเท้า ทั้งนี้ผู้ป่วยเบาหวานควรมีส่วนร่วมดูแลเท้าของตนเอง

ข้อเสนอแนะ ทีมสหสาขาวิชาชีพที่รับผิดชอบคลินิกเบาหวาน ควรจัดให้มีการประเมินความเสี่ยงการเกิดแผลที่เท้าเท้า ตลอดจนการให้คำชี้แนะด้านสุขอนามัยการดูแลเท้าสำหรับผู้ป่วยเบาหวานที่มีความเสี่ยง
