Factors Associated with Fear of Hypoglycemia

Abstract
Objective: To determine the factors associated with fear of hypoglycemia (FOH) in patients with type 2 diabetes ( T2DM) .Methods: The study was a cross–sectional descriptive analytical study, collecting the data from 345 T2DM patients who continually visited the Waing Pa-Pao hospital network in Chiang Rai province during the period of January to June 2016. The Hypoglycemia Fear Survey II (HFS-II) Thai version questionnaires used to measure FOH was composed of the behavior (HFS-B) and the worry (HFS-W) subscales. Logistic regression analysis was used to analyze
the relationship between various factors and FOH. Results: The majority of subjects were female (n=226, 65%) with an average age of 58.7±8.7 years. The mean Fasting Plasma Glucose (FPG) and glaciated hemoglobin (HbA1c) were 140.2:±44.4 mg/dl and 9.5±1.9%, respectively. The average score of HFS–B was 21.2±7.6 points from the full score of 75. The average score of HFS-W was 24.9±9.3 points from the full score of 90. Nine factors hypothesized to have a relationship with FOH were age, gender, duration of T2DM, comorbidities or complications, history of hospitalization from diabetes, 70/30 insulin use, duration of 70/30 insulin use, history of hypoglycemia and stress. It was found that only history of hypoglycemia, especially moderate to severe intensity was significantly related with HFS–B scores, compared to those with no history on hypoglycemia (OR = 2.75, P=0.002) Two factors, history of hypoglycemia at moderate to severe intensity and stress were significantly related to HFS–W score with OR=1.99, P=0.019 and OR=2.96, P=0.004, respectively (compared to those with no history on hypoglycemia). Only history of hypoglycemia at moderate to severe intensity showed the significant relationship with overall HFS score (OR = 3.08, P=0.012), compared to those
with no history on hypoglycemia. Conclusion: History of hypoglycemia and stress are two factors that show significant relationship with FOH. To strengthen the knowledge on hypoglycemia among T2DM patients, especially among those who have experienced hypoglycemia or who suffer from stress would be crucial to alleviate their FOH.
